Your Opportunity:
Reporting to the Unit Manager of the Bone and Joint Clinic, the RN Case Manager is responsible for coordinating the care of orthopedic patients within an ambulatory care setting. This role involves collaboration with multidisciplinary team members to ensure excellence in patient-centered practice and to optimize both surgical and non-surgical outcomes. Care coordination spans from initial assessment, pre-operative preparation, during admission and postoperative recovery--ensuring patients meet established clinical benchmarks. The Case Manager collaborates with physicians, interdisciplinary teams, and community stakeholders to mitigate identified risks, implement appropriate interventions, evaluate outcomes, and coordinate ongoing care. Serving as the primary access point for services and patients, the role also provides leadership to the surgical optimization team, ensuring effective coordination of pre- and post-surgical management within the outpatient orthopedic clinic. The ideal candidate is an excellent communicator with strong critical-thinking abilities. They must be flexible, organized, self-directed, and capable of working both independently and as a collaborative team member. Leadership skills are essential, along with sound clinical judgement, effective problem-solving abilities, and strong time-management competencies. The ability to work productively with colleagues and contribute to the success of the Bone and Joint team is significant.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:

Bachelor's of Nursing Degree and in good standing with CRNA. Current CRNA Registration Current BLS-HCP Certification Minimum 5 years of recent acute care nursing experience at a 0.5 FTE or higher Minimum 3 years acute care orthopedic experience at a 0.5 FTE or higher. Minimum 1 year of patientfamily orthopedic teaching required. Minimum 1 year experience using EPIC in Ambulatory/Orthopedic clinical setting Demonstrated proficiency using EPIC Experience with the current Alberta Hip and Knee Care Path Proficiency in English communication (written and verbal) Demonstrated strong interpersonal and decision-making skills. Demonstrated critical thinking & assessment skills with ability to problem solve Excellent time management and organizational skills.

Preferred Qualifications:

Master's Degree in Nursing Certification in Case Management Previous Case Management experience (within last 3 years). Previous Nurse Clinician experience (within last 3 years) Knowledge of the CPSA Guidelines Experience in South Health Campus Dept. of Surgery Orthopedics Ability to adhere to AHS Code of Conduct & Confidentiality Policy
