Your Opportunity:
The Inpatient Transition Services Coordinator (TSC) screens and assesses the needs of clients and families who cannot function independently in the community by mobilizing and managing the health and social resources required. This case management role involves assessing and/or triaging client and family needs to ensure resources to improve or maintain the client's ability to remain at home. The TSC's caseload includes clients on an acute care unit that are current home care clients, new homecare clients, clients requiring complex discharging planning, and clients that require placement in a designated living option. Working with the acute care multidisciplinary team, the TSC assists to ensure clients have the appropriate resources for discharge based on their assessed needs. Responsibilities of the TSC include: referral to appropriate community programs/services, appropriate documentation within multiple electronic systems, participation in client conferences and team meetings, developing and implementing appropriate care/service plans, coordinating service delivery across programs/settings, providing clients and families with information about programs, services, and facilities they may be eligible for, and providing guidance and education to acute care staff to ensure appropriate referral processes are followed. The TSC must be able to build strong relationships with internal and external stakeholders, be comfortable having difficult conversations, work with a diverse and complex client group, and be able to negotiate with service providers to ensure successful discharges.

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $39.58 - $54.16 per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii) as applicable.
