Your Opportunity:
The Adolescent and Young Adult Cancer Patient Navigator (AYA CPN) is a specialized oncology Registered Nurse (RN) position within AHS Cancer Care Alberta (CCA). As part of the service model for CCA, the AYA CPN is an integral member of the multidisciplinary cancer care team. This position reports to the Program Lead, Cancer Patient Navigation. The role aligns and functions within the established competencies for Navigation within CCA. Based on the requirements and standards of the Provincial Cancer Patient Navigation Program, the AYA CPN facilitates timely access to information, support and cancer services for patients and families during key transition points along the cancer care continuum. This can span from first consult to treatment and follow-up, to survivorship or palliation. At any point across the cancer trajectory, the AYA CPN in collaboration with the health care team will work to identify patients' symptoms and concerns, and tailor interventions and make referrals as appropriate to address their physical, psychological, social, emotional, spiritual, and/or practical needs. The AYA CPN works closely with key stakeholders (including adult and pediatric oncology specialists, other health care providers, supportive care services, community-based services and partners, patient representatives and AHS Cancer Care Alberta) to identify gaps and develop strategies to improve access to services to better support patients and families across the cancer journey.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Additional Required Qualifications:

Three years of recent clinical oncology experience. Experience working with/knowledge of AYA's specialized developmental and health-related experiences and needs. Experience/knowledge of the health care system and community-based resources. AYA CPNs are expected to maintain their oncology competencies within their current clinical setting as required.

Preferred Qualifications:

Nursing Oncology Certificate preferred, i.e., CON(C) / APON (and/or equivalent). Completion of CCA-AHS Cancer Patient Navigation course upon hire & within 3 months of accepting position. Community, public health nursing, or ambulatory care experience. Experience developing partnerships / collaborations within and across organizations. Literate in computer software programs including Microsoft office, ConnectCare, Netcare, Meditech, etc.
