Your Opportunity:
The East Calgary Family Care Clinic is known for its innovative approach to providing comprehensive primary care for people with complex needs. This could be any combination of chronic illnesses, mental health difficulties and addiction along with social or economic factors that negatively impact health. Reporting to the management of the East Calgary Family Care Clinic, the nurse's role is focused on the care coordination of a complex, high needs patient population using a comprehensive case management approach through development of therapeutic relationships. Patients have a wide variety of psychosocial and complex medical conditions that require a nonjudgmental approach. Understanding patient barriers in behavior change and the challenges facing patients in using the healthcare system, the RN will coordinate other team members of the medical home, community based services, or other AHS agencies to provide ongoing support to the patient according to their needs. This may include system navigation, health education, as well as other creative approaches in engaging patients in care. In conjunction with patient, family and other care providers, develops comprehensive care plans in consideration of social, financial and emotional risk factors, options, consequences of actions, health promotion, learning, comfort, safety, privacy, dignity and integrity. The Nurse utilizes knowledge & skills in assessing readiness for behavioral change using Health Change Methodology or similar frameworks to support and promote patient self-management. The nurse facilitates communication about care between the patients and families, care providers, systems and hierarchies. The Nurse will support the team in providing a nursing assessment and triage for urgent patient inquires.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0) Additional Required Qualifications:
Proven ability to work independently. Previous experience in managing patients with complex chronic diseases including mental health conditions and/or addictions. Demonstrated ability to de-escalate patients with aggressive predispositions. Minimum of three years of recent nursing experience within the last 5 years. Current experience of working in an interdisciplinary team. Demonstrated strong interpersonal communication skills. Previous experience using an electronic medical record & competence in computer usage required with ability to talk and type. Experience using Connect Care an asset. Preferred Qualifications:
Baccalaureate in Nursing. Experience in intensive case management. Experience in patient triage including phone triage. Experience working on a complex medical unit or internal medicine. Minimum of one-year recent experience in community programs within the last three years. Experience in Primary Care an asset. Demonstrated flexibility and adaptability within a complex and rapidly changing environment which may involve working with clients who present criminal or suicidal tendencies.
