Your Opportunity:
This is an on site position. There is no opportunity to work from home. Reporting to the Care Manager on the Seniors South Team within Integrated Home Care, the Registered Nurse (RN) Case Manager works as a member of an inter-professional health care team. Within the full scope of the professional practice, is responsible for assessing, care planning, organizing, reviewing, and evaluating the needs of clients and their caregivers/families requiring community-based services that fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involving provision of services and may work in multiple settings. The RN Case Manager is also responsible for providing direct nursing interventions to clients in a designated area on the Seniors South Team-Integrated Home Care Program.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $35.87/hour - $43.82/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable. Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0) Additional Required Qualifications:
Active registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Minimum two years of RN home care experience required; previous community experience in geriatrics is an asset. Excellent interpersonal, communication, and time management skills with a strong ability to work in a collaborative team demonstrating adaptability and flexibility from day to day. Proven critical thinking, decision-making, and assessment skills. Physically able to perform all assigned tasks, including manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. For example: lifting of approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common within this job. Ability to perform all duties in accordance with related legislation. Requires travel via personal vehicle throughout the city of Calgary. Recent experience in a community setting; Home Care experience preferred. Preferred Qualifications:
Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Experience working with acute clients and/or long term clients with a focus on chronic disease management. Experience in providing case management services. Canadian Nurses Association (CNA) certification in Gerontology. Experience in Home Parenteral Therapy (HPT), wound, skin and ostomy care, palliative support and client teaching.
