The Bed Hub Coordinator acts as the single point of contact for site-specific information regarding the current status of admissions, discharges and transfers. This role: requires strong clinical assessment, critical thinking & decision-making skills to accurately interpret information from a variety of sources & liaise with internal & external stakeholders. Is highly complex & requires the ability to manage competing priorities, make decisions independently & work with an inter-disciplinary team. Maintains Pathways matching system & match to appropriate queues/sites based on client needs. Reviews placement profiles & collaborates with Transition Coordinators (TCs)to ensure appropriateness for placement. Identifies appropriate matches for clients based on current waitlist management policies and guidelines & system demands. Acts as central point of contact for all transactions concerning Pathways matches & site admissions. Monitors and matches clients awaiting placement from community, various levels of continuing care homes or acute care into designated living option spaces. Applies critical thinking skills to help prevent, anticipate or resolve issues throughout the placement process. Facilitates the bed triage process based on current system demands in conjunction with the Bed Hub Manager when capacity thresholds are reached. Collaborates with sites on appropriateness of admission. Collaborates with TCs to ensure updated information is provided on a proactive basis. Provides tracking and trending data to be incorporated into reports regarding client flow. Identifies recurrent client throughput issues & develops operational plan to address issues in coordination with the Bed Hub Manager.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit Holders shall be paid $39.58 /hour - $54.16/hour. Upon being entered into the register of Registered Nurses, the rate of pay shall be adjusted to the rate of pay applicable to a Registered Nurse in accordance with AHS/UNA Article 25.02(b)(i) or (ii) as applicable.

Required Qualifications:

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)

Additional Required Qualifications:

A minimum of five years facility-based nursing experience is required. Acute care with community experience preferred. A minimum of two years of experience within the community is required. Experience in home care (or equivalent), supportive living or facility living is required. Experience as a community transition coordinator (3 years minimum required). Effective time management and prioritization skills are required. Strong clinical assessment skills and ability to review comprehensive assessments of individuals is required. Demonstrated ability to review, articulate questions to gather missing details, and provide feedback on comprehensive assessments completed by other Transition Coordinators Conflict resolution and strong communication skills are required. Must be able to work independently and within a team environment. Ability to problem solve and demonstrate critical thinking skills is required. Must be able to cope with high levels of stress, fluctuating priorities, and time sensitive deadlines. Must be able to tolerate long periods of sitting or standing as well as repetitive movements. Must be able to lift and carry 5-10kg regularly.

Preferred Qualifications:

Knowledge of computer applications such as Microsoft Apps, Pathways, interRAI Tools, Connect Care and e-People is preferred and an asset. Bed management experience is an asset.
