Job Summary:
Working in a fast-paced, dynamic outpatient environment focused on meeting the patient needs of the
Sam & Ida Ross Memory Clinic and the Virtual Behavioural Medicine (VBM) program
, the successful candidate(s) will be a member of an interprofessional team that provides a comprehensive evidence- based approach in managing/supporting the care needs of people with complex neuro - degenerative brain disorders. The
Sam & Ida Ross Memory Clinic
provides tertiary level specialist assessment and treatment for people with mild to moderate cognitive impairments and dementia. Problems may include memory loss, cognitive deficits, speech and language disorders due to neurodegenerative brain disease. The
VBM
program is a rapid access tertiary level specialist program that focuses on treating complex neuropsychiatric behavioural symptoms of advanced dementia for patients living in the community, in Long Term Care and in acute care settings, where existing treatment resources have not been successful. The Austin Centre focuses on innovative care through research embedded in clinical practice and is a leader in the advancement of dementia care and neuro-behavioural care working collaborating with health system partners across the province. These positions report to the Austin Centre Clinical Manager.
Responsibilities include but are not limited to:
Provide evidence-based clinical assessments for neurodegenerative brain disorders and participate in program approved research embedded in clinical practice
Possess sound knowledge and skills, acts as clinical expert and resource and provide quality client/family centered health services
Triage, coordinate and facilitate access to clinical services and resources
Provide supportive clinical counseling that promotes and supports the physical, psychosocial, and spiritual well-being of clients, their families, and caregivers
Plan, deliver and evaluate nursing interventions, including medications as well as treatments which fall within the RN's scope of practice
Demonstrated ability to practice in a self-directed way, taking professional accountability for leading, organizing, providing and delegating care activities for clients and families
Identifies and participates in approved quality improvement initiatives
Awareness of community services to promote client independence and self-management
Participate in the advancement of the clinic by mentoring students and novice practitioners
Work collaboratively within an interprofessional team
Qualifications include but are not limited to:
Current registration, in good standing, from the College of Nurses of Ontario ('CNO') is required
Current Basic Cardiac Life Support ('BCLS') certificate, Level C - Automated External Device ('AED') is required
BScN required, Master's preferred
CNA Certification in Neuroscience or Geriatrics required or in progress
Knowledge of memory disorders, dementia care, brain anatomy and physiology required
Experience working with clients with memory disorders due to neurodegenerative diseases and psychogeriatric symptoms of dementia required,
Highly developed clinical assessment skills and expertise in the care of seniors with complex medical, neurodegenerative, psychogeriatric, cognitive and social needs required
Proficiency with medication practices, side effects and current treatment modalities required
Experience administering cognitive tests such as the Mini-Mental Status Examination (MMSE), Montreal Cognitive Assessment (MoCA), and Toronto Cognitive Assessment (TorCA) is preferred
Demonstrated knowledge and experience in providing virtual healthcare, use of OTN using ONE ID, ZOOM
Exceptional client service skills
Excellent interpersonal, communication, problem solving and organizational skills
Excellent verbal and written skills
Demonstrated ability to manage competing demands while setting priorities and meeting deadlines
Demonstrated commitment to professional development and continuous learning
