Job Summary


The Englehart and District Family Health Team is an interdisciplinary team of healthcare professionals dedicated to providing comprehensive, patient-centered, team-based primary healthcare services. These services include health promotion, disease prevention, preventative care, and chronic condition management for the town of Englehart and the surrounding areas. We are looking for a compassionate and skilled Registered Nurse (RN) to join our primary care team, providing essential care to patients. The RN will work alongside an interdisciplinary team to deliver comprehensive nursing care, support chronic disease management, and address the social determinants of health.

Responsibilities



Provide primary care nursing services to patients, including health assessments, medication & immunization administration, wound care, and chronic disease management. Collaborate with physicians, nurse practitioners and other healthcare professionals to develop and implement individualized care plans, monitor patient progress, report changes in health status, and adjust care plans as needed. Conduct health education and promotion activities to empower patients in managing their health and preventing disease. Possess multi-tasking skills for delivering efficient primary care, with the ability to prioritize and adapt in a fast-paced, constantly changing work environment. Advocate for patients as warranted, facilitate connections to community support, and assist them in navigating the healthcare system effectively by ensuring they understand the resources and steps needed in their care. Maintain clinical records and documents, patient assessment findings, goals, treatment plans and interventions in accordance with the EDFHT policies and procedures and as per guidelines established by the College of Nurses. Contribute to the development and review of policies, protocols, and procedures Protects patient confidentiality in accordance with legislation, EDFHT policies, and standards established by the College of Nurses and ensures adherence to PHIPA.

Requirements



A Baccalaureate Degree in Nursing. Current registration in good standing with the College of Nurses of Ontario as a Registered Nurse. 2-3 years of clinical experience in primary care, community health, or acute care setting. Strong physical assessment skills to support patients during appointments and sound theoretical knowledge. Possess excellent communication and interpersonal skills, with a strong commitment to patient advocacy and the ability to work both independently and collaboratively in an interdisciplinary team. Current BCLS certification. Proficiency in the use of computers and information technology, including experience with electronic medical records (EMR), Practice Solutions preferred. Participate in ongoing professional development to maintain and improve skills and knowledge, benefiting the team and patients. Ability to speak and write in English, the ability to communicate in French to be considered an asset.

Additional Job Requirements



Must adhere to all EDFHT Policies and Procedures. Must be legally entitled to work in Canada. This position is in Englehart, ON, and requires working in the office and on-site.
