As a key member of Connexion Family Health Team (CFHT), and an integral part of an interdisciplinary health care team, you will deliver Dietitian care and education in accordance with FHT policies and professional practice standards. You will perform routine dietitian duties, and ensure that the needs of our patients are met.
Your primary roles are to work in partnership with physicians and other health care providers to assess patient needs, develop and implement nutrition plans with patients, monitor patient progress, and provide education and advocacy. You will work with physicians on referrals and collaboration as they relate to diet and nutrition.
As a member of a multidisciplinary team, you will contribute to health promotion and illness prevention strategies and develop specialized nutrition therapy, provide education and dietary counselling, rehabilitation and support strategies to address specific nutrition related illnesses.

Strong verbal and written communication skills and the ability to adjust language and terminology to suit the individual patient or patient group

Language Skills
Bilingual (French and English) for verbal communication is preferred.
Education and Credentials
Minimum Education: Completion of an accredited bachelor degree in Foods and Nutrition and an accredited Dietitian Internship
Registration and Membership with the College of Dietitians of Ontario
Relevant Experience
A minimum of 3 years of experience working in the field would be considered an asset

Hours of Work - Monday to Friday 8:00 a.m. - 4:00 p.m., on-site, 37.5-hour work week with the possibility of one evening shift (11a-7p) per week
