Member of the health care team responsible for provision of nutrition care to patients who are identified as nutrition care priorities.
Within this role the employee is accountable for contributing to the delivery of the KHSC strategy. As an employee, one must demonstrate an awareness of and be responsible for actively promoting and supporting patient and family centered engagement and care in all we do.
PRIMARY RESPONSIBILITIES & DUTIES
Patient Care Responsibilities
Eligible for College of Dietitians of Ontario (CDO) membership and practice within the professional, legal, and ethical standards outlined for Dietitians in Canada.
Provide nutritional care to patients who have been referred and/or, in an inpatient setting, screened as moderate - high nutritional risk. This would include assessment, development of a nutrition care/education plan, implementation, documentation, monitoring, and altering nutrition care plan as needed.
Act as a consultant in normal and therapeutic nutrition in specific areas of practice, ensuring that patients/clients receive appropriate nutrition care.
Comply with organizational, program, and departmental policies and procedures and standards of care.
Take responsibility to ensure that practice is evidence based and patient centered.
Use effective communication and counseling skills to ensure patient and caregivers are informed of the nature and possible effects of the nutritional intervention.
Document accurately and efficiently within the patient's medical record.
Provide direction and supervision to the dietetic technicians participating in the care of moderate-high nutritional risk patients.
Program and Departmental Responsibilities
Attend monthly practice council meetings
Attend and/or present at monthly practice education sessions
Support the advancement and dissemination of nutrition related knowledge and skills through participation in program specific and/or corporate wide interprofessional or student education
Participate in program specific nutrition research activities
Participate in program specific staff meetings
Participate as an active member of the health care team through inter professional patient care rounds, quality and continuous improvement and evaluation activities
Act as a clinical nutrition representative on program specific clinical practice teams, committees, and working groups
Participate in clinical nutrition and food service quality assurance/improvement activities, such as: evaluation of food quality, patient care projects, menu planning or modifying, and nutrition standards of care, etc
Organizational Responsibilities
Complete workload measurement statistics, program hours, and time sheets
Complete KGH Learning Management System corporate training courses as assigned
Act as clinical nutrition representative on corporate committees and project teams
Educational/Professional Responsibilities
Maintain a high standard of personal competence through continued education and an ongoing critical evaluation of professional experiences
Support others in the pursuit of professional goals and life long learning by providing education, direction, and mentorship to dietetic technicians, dietitians, interdisciplinary staff and students
Support training and education of future members of the profession by mentoring dietetic interns
Maintain/develop knowledge and skills in assigned patient care area(s)
Maintain competency relevant medical directives and delegations

EDUCATION, QUALIFICATIONS & EXPERIENCE:
College of Dietitians of Ontario approved academic preparation from an accredited Canadian university program in foods & nutrition or equivalent and practical training in an accredited practical program in Canada or equivalent.
Must be eligible for membership with the College of Dietitians of Ontario.
One (1) year recent related clinical work experience in an acute care setting preferred
Eligibility for Diabetes Educator Certification an asset
Excellent communication and time management skills
Excellent interpersonal skills
Ability to cope with fast paced situations in patient care areas
Proficient with computer applications e.g. Excel, Word, Windows, Internet.
Able to work independently and efficiently
Able to prioritize tasks and be flexible
Proven ability to attend work regularly.
Satisfactory criminal reference check and vulnerable sector search required.
