Position Summary:



The dietician is to develop and implement nutrition programs to promote healthy diet for adults at high risk or living with type 2 diabetes; promote wellness, and maintenance of healthy lifestyles through a complete consultative approach; work within the administrative procedures established for this position; represent and promote the health service interests of the Mohawks of the Bay of Quinte with an indigenous focus; work in a courteous, cooperative, positive manner; and, provides information as requested.

Reporting:



This position will report to the Kenhte:ke Diabetes Education Program Manager. This position will have no direct reports.

Summary of Main Responsibilities:



? With an indigenous focus, develop and deliver effective individual and group nutrition education and programming for the prevention and management of type 2 diabetes by:

o Improving the nutritional knowledge of adults who are at risk of, or 2 diabetes;

o Providing diabetes education services, in conjunction with the service teams to ensure that the Diabetes Education is accessible and meets the needs of clients with diabetes, their family and or support system;

o Responsible to see those at risk for diabetes, weight loss needs and any other health related issues to diabetes;

o Providing clients with practical advice and assistance with nutrition and Healthy living/lifestyle and Diabetes prevention and management;

o Providing clinical nutrition assessments to develop care plans with clients and care providers;

o Developing, implementing and evaluating a range of prevention and management programs;

o Understanding and addressing barriers that the community and individuals face in maximizing their positive health status and gaining access to health services;

o Collecting, compiling and providing statistics that fulfill funding requirements; and

o Collecting statistics for reporting and completing other reporting.

? Maintain individual client health files/documentation according to the established standards of the MBQ Community Wellbeing department by:

o Ensuring the safety, security, proper storage, confidentiality, and disposal of information;

o Document individual and group records to appropriate clinical standards; and,

o Preparing files, documents, forms and correspondence as necessary.

? Work within the Community Wellbeing team promoting positive client outcomes by:

o Providing high quality, professional services as part of a multidisciplinary team;

o Reviewing information on local health trends, such as mortality, morbidity epidemics, local conditions and attitudes; cultural, social, economic and environmental factors;

o Participating on committees and in workshops that relate to diabetes health or as requested;

o Identifying issues and options and selecting the most appropriate solution;

o Discussing priorities to meet urgent or changing needs;

o Managing projects, etc. to achieve work plan outcomes;

o Implementing appropriate strategies to promote equitable access to services for all members of the community;

o Performing work tasks in accordance with the MBQ Health and Safety policy to ensure the health and wellbeing of self, other staff and clients; and,

o Completing administrative tasks as needed for the effective maintenance of work.

? Health and Safety:

o Promoting and following all MBQ Health and Safety policies and procedures

? All other duties as required

Requirements:



? Bachelor of Science in Nutritional Science with experience in Diabetes education preferred

? Certified or willing to obtain certification for CDE in the next 2 years.

? Member or eligible for membership with the Ontario College of Dietitians

? Good knowledge, preferable gained through related experience and training in the delivery of nutrition services

? Experience in a First Nations setting is an asset

? Combination of training, education and experience may be considered (at the discretion of the organization)

? Must provide a satisfactory Vulnerable Sector Check (documentation will be required if successful applicant)

? First Aid and CPR an asset (and must be willing to take the training if hired)

? Valid Driver's License and vehicle

Knowledge, Skills and Abilities:



? High level of knowledge and sensitivity to Indigenous issues; culture and traditional foods

? Exhibits a high degree of initiative and self-direction; good analytical, organizational, verbal and written communication skills

? Ability to work with tact and discretion; high level of public relation skills and good project management skills.

? Able to develop and deliver workshops during operating hours, weekends and /or evenings as needed and directed. Develop and deliver information sessions, and other presentations to individuals and groups of clients at age-appropriate levels and in clear terms for the general populations

? Knowledge and understanding of indigenous culture and practices

? In-depth knowledge of nutrition and prevention and management of type 2 diabetes

? Advanced knowledge with computer programs (Word, Excel, Outlook, database, EMR and more)

? Ability to work as an individual and as part of a team

? Excellent organizational, time and file management skills

? Strong attention to detail

? Ability to deal with conflict

? Strong ethics and integrity

? Excellent problem solving and decision-making skills
