KCPCN/ VFC is a collaborative, patient-focused primary care practice dedicated to providing quality healthcare services to our community. We are looking for a reliable, organized, and compassionate Referral Coordinator to join our team. This role plays a vital part in ensuring seamless coordination between patients, specialists, and healthcare providers.
Position Summary:
The Referral Coordinator (RC) is responsible for coordinating all patient referrals and specialist appointments, maintaining follow-up communication, and ensuring accurate documentation of referral activities. The RC reports directly to the Clinic Manager and may assist with other administrative duties as needed to support clinic operations.
This position requires strong attention to detail, excellent communication skills, and the ability to manage multiple tasks efficiently in a fast-paced clinical environment.
Key Responsibilities:
Prepare and send referral letters to specialists and external clinics.
Book and follow up on all specialist appointments, ensuring patients receive timely updates and instructions.
Maintain organized and up-to-date records of all referrals (Accuro EMR, VFC Landing Site RC documents, and physical binder).
Communicate professionally with patients, physicians, and external offices.
Handle patient inquiries and conflicts with professionalism, documenting any incidents as required.
Ensure confidentiality and compliance with Alberta's Health Information Act (HIA).
Maintain cleanliness and organization of the workstation area.
Support other clinic operations as directed by the Clinic Manager, including:
Assisting with call-ins, unlinked and linked documents
Picking up and sorting mail
Ordering office and kitchen supplies
Completing bank deposits as assigned
Performing light housekeeping or restocking tasks as needed
Referrals the Coordinator Will Manage
All specialist appointments
Holter monitor and stress tests
Pain management clinic referrals
Occupational therapy and physiotherapy
Wound care referrals
Hip and knee programs
Endoscopy and colonoscopy ("scopes") referrals
Comprehensive Breast Care Program
EMG studies
Mental health referrals
Sleep studies (excluding level 3)
Bariatric program referrals
Cannabis clinic referrals
Qualifications:
Previous experience in a medical office or healthcare setting preferred
Knowledge of Accuro EMR or similar electronic medical record systems an asset
Strong administrative, organizational, and communication skills
Ability to work independently and as part of a team
Professionalism, discretion, and a commitment to patient confidentiality
Flexibility to assist with other clinic duties as required
Additional Information:
The Referral Coordinator may be cross trained in other administrative roles to ensure adequate coverage during absences and to support long-term clinic stability.
Reports directly to the Clinic Manager.
