St. Joseph's Health Centre, Unity Health Toronto has a 30 bed Transitional Care and Reactivation Care Unit located at Humber Church site (200 Church St.; near Jane Street south of the 401). The Reactivation Care Unit provides care for adult and geriatric patients who require therapy, reactivation and transition planning in order to transfer to the community and/or other supportive environments. We are now recruiting a dynamic inter-professional team to deliver care to, and with, the patients in this program!
This unit focuses on reactivation and transition planning for patients who are awaiting transfer to the community and/or other supportive environments. Working as a part of a dedicated, inter-professional team, you will deliver patient centred care designed with best practice principles. This team works closely with patients, families, and caregivers to provide reactivation therapy with the goal of enhancing and optimizing patient's functioning and engagement in daily routines.
Position Summary:
The Recreation Therapist provides services which utilize assessment, treatment, education, and recreation participation to enable persons with physical, cognitive, emotional and/or social limitations to acquire and/or maintain the skills, knowledge and behaviors that will allow them to enjoy their leisure optimally, function independently with the least amount of assistance and participate as fully as possible in society. The Recreation Therapist will provide these services following the Standards of Practice for Therapeutic Recreation in Ontario (TRO) and/or will be a Certified Therapeutic Recreation Specialist (CTRS). Therapeutic Recreation is directed toward functional intervention, leisure education and participation opportunities, which includes community integration. These processes support the goal of assisting the individual to maximize independence in leisure, optimal health and promote the highest possible quality of life.
Qualifications:

  • Bachelor of Arts Degree in Therapeutic Recreation from a recognized University, or health-related undergraduate degree and Georgian post-graduate certificate in Therapeutic Recreation.
  • Minimum 2 years' experience working within a hospital, rehabilitation program or Long Term Care, as a Therapeutic Recreationist
  • Knowledge of Therapeutic Recreation assessment and therapeutic skills, interventions, various leisure activities, activity analysis, community integration resources and planning, individual and group facilitation techniques, protocol development, documentation, and volunteer utilization.
  • Knowledge of various Therapeutic Recreation assessment tools and techniques.
  • Knowledge of an individualized and systematic process to gather, analyze and interpret data.
  • Understanding of documentation and outcome measures.
  • Knowledge of Therapeutic Recreation program evaluation measures and outcome measures.
  • Knowledge of current Therapeutic Recreation models and Standards of Practice for provincial and national affiliations.
  • Knowledge of current Therapeutic Recreation literature, resources, standards, trends and research.
  • Possess understanding of the needs of adult and geriatric populations, including but not limited to disease and disability related to setting.
  • Understanding of the physical, social, emotional, cognitive and spiritual impacts of Therapeutic Recreation and how they relate to overall health. Knowledge of relevant adaptive leisure equipment for adults with disabilities.
  • Ability and experience required to work cooperatively and collaboratively within an interdisciplinary team setting.
  • Excellent interpersonal and communication skills.
  • Knowledge and experience in research development and applications.
  • Basic computer knowledge and skills.
  • Demonstration of on-going professional development activities.
  • Ability to select, administer, implement, score, analyze and interpret standardized and non-standardized assessments.
  • Ability to select goal-directed and outcome-oriented interventions aimed at maintenance and/or enhancement of individual leisure status and functional level.
  • Ability to write comprehensive program protocols.
  • Ability to convey program protocols, goals and objectives, and criteria to patients/clients and interdisciplinary team members.
  • Ability to adapt to changing environments.
  • Ability to maintain Therapeutic Recreation documentation.
  • Ability to evaluate individual/group process and apply modifications as necessary.
