Reaching Home Coordinator
to support community members in Moosonee, Ontario who are experiencing or at risk of homelessness. This is a front-line, community-based position focused on providing essential supports, advocacy, and culturally grounded services to some of the community's most vulnerable individuals.

Key Responsibilities
The Reaching Home Coordinator will:
Provide frontline support to individuals facing or experiencing homelessness in Moosonee
Assist clients with
food security supports
, including access to food programs and resources
Support all
housing-related paperwork
, including applications, referrals, and documentation
Conduct
data entry
and maintain accurate records within the Homeless Individuals and Families Information System (HIFIS)
Offer
crisis intervention
and de-escalation support when required
Collaborate with local service providers, shelters, and community partners
Advocate for client needs and help connect individuals to culturally appropriate supports
Participate in program planning, reporting, and evaluation as required
Qualifications
We are seeking a candidate with education and experience aligned to the needs of this role. Preferred qualifications include:
Diploma or degree in
Social Work
,
Mental Health and Addictions
,
Indigenous Studies
, or a related human services field
Experience working with Indigenous communities (asset)
Understanding of trauma-informed care, harm reduction, and homelessness support services
Ability to work independently in a remote community setting
Strong communication, documentation, and organizational skills
Valid Class G driver's license
