Job Summary:To deliver and coordinate public health nursing services to individuals, families, groups and communities and to participate in the designing, planning and evaluation of public health programs and services related to Multi-Tenant Houses (Rooming Houses). Participates in the development of work plans based upon divisional operational plans. Collaborates and liaises with community groups to facilitate service delivery. Services may include, but are not limited to services such as maternal/child care, (e.g., prenatal, postnatal, postpartum breastfeeding support, parenting), seniors, communicable disease control and sexual health clinic services, chronic disease prevention (e.g., nutrition, physical activity, tobacco use prevention, heart health, cancer prevention), injury/substance abuse prevention, mental health issues and sexual health promotion.Major Responsibilities:
  • Provides public health nursing service for individuals, families, groups and the community according to the program-specific policies and procedures and program and operational plans of MLS, the Community Health Nursing Standards of Practice and the standards of the College of Nurses of Ontario.
  • Carries out work in Multi-Tenant Houses and in various community locations as required and works collaboratively with MLS Bylaw Enforcement Officers to ensure Multi-Tenant Houses across the City of Toronto are operating according to the new regulatory framework.
  • Makes initial service response assessment; provides health information, education and counselling to the community. Directs clients to community resources. Liaises with program areas and area office staff to ensure communication of relevant information and issues.
  • Carries out independent home visits and/or clinical services. Assesses individual/family status and assists the individual/family in developing and achieving health goals. Counsels, consults, collaborates and advocates for clients. Provides appropriate public health services and liaises with and refers to other health professionals and agencies as needed.
  • Delivers interventions to individuals and families in high-risk situations and acts as the service coordinator for these families. Advises/reports to appropriate authorities (e.g., Children\'s\' Aid Society) as required and within the bounds of related legislation.
  • Maintains individual/family/group/client/school records and necessary documentation according to the policies and procedures of MLS as well as the Standards of Practice set out by the College of Nurses.
  • Collaborates and consults with community groups, coalitions and agencies such as schools, community centres, or workplaces and early years centres to assess health needs.
  • Facilitates community partnerships, and liaises with community agencies, groups or organizations to address community health issues and access to public health programs and services. Also provides consultation on policy development and implementation based on appropriate legislation and best practices.
  • Mentors, and provides orientation and training to new staff, community professionals, students and peers.
  • Collaborates and liaises with other professionals, community agency staff and City of Toronto staff to identify health needs and participates in the development of community partnerships and action plans to meet those needs.
  • Prepares, submits and presents activity reports, data and other reports, e.g., such as documentation, time and activity data collection, funding applications, professional reports (journal articles, conference abstracts), and requests for proposal for managerial approval. Participates in the generation of Ministry of Health reports, Requests for Proposals, contracts, sole source letters, and quotes for managerial approval. Contributes to reports needed for external funding sources (e.g. Heart Health) for managerial approval. Participates in tracking and processing budget, honoraria, cash advances and petty cash.
  • Appears and gives evidence in court and inquests when subpoenaed in consultation with management and legal services.
  • Presents and participates in professional conferences and workshops. Makes presentations to other health professionals/colleagues.
  • Participates in the development of policy, procedures and protocols and updates resources for managerial approval.
  • Participates in the planning, development, implementation and evaluation/research of public health programs and initiatives, which include advocacy for healthy public policy, social marketing campaigns, community mobilization, health education and skill building.
  • Participates in the divisional planning process including the development of work plans based on operational program plans.
  • Reviews program-specific community grant applications using specific program-based criteria and assists with selection process.
  • Participates in the co-ordination of projects as determined by program specific operational plans.
  • Provides direct care to clients in clinical settings. Provides clinical services under medical directives in certain situations (e.g., immunizations, physical examinations including but not limited to pelvic examination, Pap smear, and sexually transmitted disease (STD) testing, treatment based on positive lab result for specific infections, manages anaphylaxis and provides immediate care/action in emergency situations. Performs venipuncture, orders laboratory tests, provides prescription for and/or dispenses medications as per clinic policies and procedures e.g. STD medications, emergency contraceptive pill, oral contraceptives). Reviews laboratory results and follows up as per clinic policies and procedures.
  • Reviews incoming lab slips reporting communicable diseases, provides assessment, counselling, education and referral to people diagnosed with a communicable disease/completes partner notification.
  • Ensures the collection of appropriate clinical specimens and forwards to laboratory for analysis. Receives, and interprets test results and follows up on lab test results, prescribed medication and dosage with client and doctor and liaises with other health professionals as necessary. Ensures treatment is adequate for the disease. Ensures compliance with treatment for a reportable disease. Identifies, tests and provides follow-up of contacts in all appropriate settings including high-risk settings.
  • Participates in surveillance to determine possible causes of outbreaks at the area level and liaises with institutions to ensure proper outbreak management practices.
  • Investigates complaints relating to infection control issues in hospitals, Long-Term Care Facilities, Day Nurseries, Personal Services Settings and other community settings as appropriate, interprets results of the complaints and takes appropriate action.
  • Inspects licensed premises, other facilities and premises for compliance with relevant Municipal, Provincial and Federal legislation e.g. Lodging Homes.
Key Qualifications:Your application must describe your qualifications as they relate to:
  • A Baccalaureate degree in nursing from a recognized university and a valid licence from the College of Nurses of Ontario.
  • Experience in the provision of harm reduction, drug treatment/addiction counselling, methadone, social, health, housing, and HIV/AIDS-related services.
  • Experience working in an outreach capacity with people who use drugs, people experiencing mental health issues and/or people who are homeless or underhoused.
  • Possession of a valid Ontario Driver\'s License Class "G" and access to a vehicle and must qualify for the City Driver\'s permits and requirements.

  • Ability to conduct home visits and establish effective therapeutic relationships with an equity lens for clients coping with social determinants of health issues.
  • Ability to exercise nursing judgment with a high degree of autonomy.
  • Ability to handle ambiguity and to function effectively while dealing with competing priorities including client-centered care, legislation, community health and safety and time management.

  • Ability to work flexible hours which may include overtime, shift work including evenings, and weekends may be required.
  • Ability to drive to various locations throughout the City of Toronto in a timely and efficient manner.
