We are currently seeking a highly motivated health professional to contribute to our organizational mission of working with our communities to promote healthier communities for all.
Reporting to the Program Manager, the Public Health Nurse will be primarily supporting the Healthy Babies Healthy Children Program and infant feeding clinics. The incumbent will work cooperatively with agency staff, community partners, and individuals to provide the leadership and nursing expertise required for program planning, delivery, and evaluation.
Responsibilities:

Shall posses strong clinical assessment skills of infants and children.
Demonstrated ability to work with and engage marginalized and high-risk populations in a professional capacity.
Shall have understanding and knowledge of infant and child development.
Understanding of all infant feeding principles that support health growth and development.
Conducts thorough nursing assessments and provides clinical support during infant feeding clinic appointments.
Shall posses understanding of the principles of breastfeeding assessment and management.
Provides information to families on pregnancy, positive parenting, healthy child growth and development, providing a safe, secure and nurturing environment, and problem-solving skills.
Ensures accurate, timely, and ongoing documentation of all client/family interactions--including home visits, telephone communications, and multidisciplinary case conferences in alignment with program timelines and agency policies and procedures.
Applies clinical expertise to assess the client's health status facilitating collaborative planning, implementation and evaluation of appropriate nursing interventions.
Delivers direct nursing care, health education, counselling and referrals for health-related concerns to ensure case coordination, as required.
Maintains documentation records in accordance with the standards of the College of Nurses of agency policy and procedures.
Administers prescribed medication in accordance with the standards and guidelines of the College of Nurses of Ontario, following established agency medical directives, and adhering to agency policy and procedures to ensure safe, effective, and authorized medication practices.
Provides supports for the essential responsibilities/duties of fellow public health nurses during their absence to ensure continuity of care and service delivery.
Understands and incorporates the social determinants of health in programming.
Works collaboratively as a multidisciplinary team member to meet established goals and objectives.
Demonstrates strong interpersonal skills including effective problem solving, conflict resolution, negotiation, and mediation skills.
Participates in the orientation of staff, students, and others as may be required for the position.
Requirements:

Recognized Bachelor of Science in Nursing or Bachelor of Nursing Degree.
Current certificate of competence with the College of Nurses of Ontario.
Current CPR level C certification.
Minimum of one-year full time equivalent nursing experience (within the last three years) in child and family health, preferably in the Healthy Babies Healthy Children program.
Experience in working with and assisting high-risk families with infants and/or young children.
Demonstrated knowledge of positive parenting skills required to support families in challenging life situations using a trauma informed approach.
Ability to recognize and manage personal resiliency/response related to information collected or witnessed while working in the HBHC program.
Proficiency in French (Oral/written) is an asset.
Competency to work under the authority of Medical Directives.
Demonstrates advanced proficiency in conducting comprehensive nursing assessments, implementing evidence-based interventions, and evaluating client outcomes to support health and well-being.
Demonstrates effective communication skills and applies professional conflict resolution strategies to support collaborative, client-centered care within interdisciplinary teams.
Demonstrated computer proficiency.
Maintains current Ontario driver's license and has access to a reliable vehicle.
Satisfactory Police Records Check (Vulnerable Sector) is a condition of employment.
