Job Description


The social worker will work collaboratively with the physicians and other allied health care providers of our team by providing care for clinic patients and accepting referrals from, as well as consulting with the physicians as necessary. Provision of primary care will be within the scope of the Social Worker (SW) and she/he will play a key role in developing programs with the clinic to enhance patient care. This role functions autonomously and collaboratively with patients, physicians, and the interdisciplinary team to support the Vision, Mission, and Core Values of WMC FHT and to ensure that organizational accountability systems are implemented. RESPONSIBILITES: Responsible for making clinical decisions and exercising judgement to ensure best possible care is provided within available resources;
Provides counseling services, with particular regard to individual and family issues and decisions that impact the health of the patient and family
Direct knowledge and skill in Family Counseling, Handling Work Related Stress/Conflict, Couples Counseling, Child & Youth Counseling and Eating Disorders
Works within the primary care model of therapy, ensuring seamless transition to community resources as appropriate;
Is knowledgeable of the community and the resources available, especially in the areas of crisis services, Mental Health, and Long Term Care;
Knowledge and skill in group work including cognitive behaviour therapy;
Will participate in data management and measurement of quality improvement indicators of patient outcomes;
Communicates results of measurements that are easy to use and understand, looking for patterns that could assist with improvement;
Acts as an advocate for patients and their families in matters affecting health;
Liaises with physicians and other team members as required;
Effectively uses the Social Work standards of practice and the therapy process in assessing, planning, implementing, and evaluating the care of the patient;
Maintains own continuing education to enhance professional knowledge and growth;
Participates and collaborates with other Family Health Team members and health care providers in provision of patient care QUALIFICATIONS: **Primary Care Experience is an Asset Master\xe2\x80\x99s degree in Social Work in with 3-5 years clinical experience
Registered with the Ontario College of Social Workers
Ability to work independently and as part of a collaborative team
Direct knowledge and skill in Family Counseling, Handling Work Related Stress/Conflict, Couples Counseling, Child & Youth Counseling and Eating Disorders
Strong assessment skills and knowledge of community resources
Experience with delivering specific therapeutic approaches such as cognitive behaviour therapy, substance abuse treatment etc.
Understanding of discharge planning and transitions implementation
Experience in assessment and counselling in a primary care setting
Experience in program development, facilitation and evaluation
Strong interpersonal, organizational and communication skills (both written and verbal)
Ability to work effectively within an interdisciplinary team and across multiple services
Experience in working with families of patients
Should be aware of conditions and measures that may affect the safety of patients, students, employees, volunteers, physicians and visitors
Proficient with an EMR system for charting an asset
Ability to prioritize, manage time effectively and be flexible in a very active work environment ***This is a Permanent Part-Time position with a pro-rated salary. Part-time hours: 20 per week Salary: $65,000.00-$73,000.00 per year Schedule:

  • 8 hour shift
  • Day shift

  • Bachelor\'s Degree (required)
Licence/Certification:
