Are you interested in exploring a new Registered Practical Nurse (RPN) role created at Nucleus that will bring further expertise and knowledge from a nursing perspective to our overall clinical teams of personal support workers, clients, consumers and families along with other system partners. The Administrative Clinical Supervisor role will help to support our teams in the community, navigate complex situations that may occur over the evening and overnight hours to support the Emergency Department (ED) Diversion in a way that is creative and solution focused with our teams together.

POSITION SUMMARY


Reporting to a Senior Manager role to begin, the RPN -Clinical and Administrative Lead provides high-quality nursing medical care to clients in a community setting. The RPN works collaboratively with an interdisciplinary team to promote client health, independence, and well-being. Care is delivered in accordance with the organization's policies, professional standards, and applicable legislation.

POSITION RESPONSIBILITIES

Conduct virtual/phone assessments and update individualized care plans in collaboration with clients and their families as required in the moment to potentially divert a clinical situation escalating to a visit to the emergency department. Proactively navigate with clinical expertise and their knowledge to assess, discuss and work collaboratively with healthcare professionals to support client focused care. Provide direct nursing and supportive care, including wound care, medication administration, chronic disease management, and health teaching. Actively engage in care in clients' homes, inclusive of lifts, transfers with team support. Monitor and evaluate client progress, adjusting care plans as necessary appropriately documenting and following up upon shift exchange to communicate changes in client conditions as required. Accurately document all assessments, care provided, and client outcomes in electronic medical records (EMR). Communicate changes in client condition to the Care Team, and other relevant team members. Participate in regular case conferences and interdisciplinary team meetings. Provide health education to clients and their families to promote self-management and prevent illness. Advocate for clients to ensure access to appropriate community resources and support services. Work collaboratively with Personal Support Workers (PSWs), Social Workers, Physiotherapists, and other health professionals. Participate in staff training, orientation, and mentoring as required. Follow all infection prevention and control protocols. Ensure compliance with the Regulated Health Professions Act, Nursing Act, and other relevant legislation. Report incidents, safety concerns, or potential risks promptly.

SKILLS AND QUALIFICATIONS

Current registration as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario (CNO). Minimum of 3 to 5 years of experience in a community or similar healthcare setting. Proficiency in wound care, palliative care, and chronic disease management is an asset. Current CPR and First Aid certification. Proficiency in using electronic medical records (EMR) systems- Alayacare specifically is an asset. Valid driver's license and access to a reliable vehicle for home visits and travelling within the regions. Strong clinical and critical-thinking skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Strong organizational and time-management skills. Cultural competency and the ability to provide care to diverse populations.

WORK ENVIRONMENT

Work is performed in a variety of settings not limited to client homes, community settings, hospital, clinic locations and the organization's office. Ability to work routine days (8 hour) / nights, 12 hour shifts, with flexibility to support evenings, weekends, and holidays as required. Must be able to travel to various locations within the service area. Occasional exposure to challenging environments and situations.

SPECIAL CONDITIONS

A Criminal Record Check and credential verification will be conducted. Some evening, weekend, and/or holiday work may be required. Nucleus Independent Living has a scent-free environment policy. Vaccination will be a condition of employment for new hires, except when impossible due to legitimate, established exceptions. The vaccine must be a two-dose COVID-19 vaccination series approved by Health Canada.
