COMPENSATION: $52,535 - $63,260 Annually Prorated

To help fulfill its mission, LOFT is hiring a Psychogeriatric Case Manager.

JOB SUMMARY:

The Psychogeriatric Case Manager provides case management services to vulnerable older adults and seniors living in an Enhanced Specialized Assisted Living Program. Provides case management services to support the physical, cognitive, and mental health needs of vulnerable older adults and seniors.

WHAT YOU WILL DO:

  • Assist clients in the development and implementation of a care plan tailored to the client\xe2\x80\x99s needs and issues designed to improve their quality of life. Identify and refer to resources specific to those needs and issues.
  • Act as a liaison and client advocate among those involved with the client\xe2\x80\x99s care including family, other care-givers, medical and other multi-disciplinary providers.
  • Conduct assessments such as RAI and OCAN and provide timely and accurate record keeping and reports as required.
  • Provide supportive counselling or referral and linkages to health care, programs or service providers.
  • Provide crisis management/intervention and assist in the development of crisis plans for clients.
  • Develop respectful and trusting relationships with clients using Recovery and Psychosocial Rehabilitation principles.
  • Maintain strict confidentiality and follow all other required policies and procedures.
  • Liaise, collaborate and coordinate with other professionals, service providers and partners.
WHAT YOU BRING:

Must Haves
  • Completion of a degree in social services/human services or equivalent.
  • Minimum 3 years of experience in providing support to vulnerable seniors and older adults with special needs including mental illness, addictions and severe social isolation.
  • Completion of Gerontology certification is an asset.
  • An understanding of harm reduction, recovery and psychosocial rehabilitation principles, the therapeutic relationship, and an ability to come to an agreement with clients on goals/priorities.
  • Creativity and flexibility in approaches such as crisis intervention strategies, problem solving abilities expressed through strong interpersonal skills.
  • Experience in group counselling, responding and assessments skills
  • Good oral and written communication skills and computer literacy, including MS word and Excel.
  • Knowledge of community resources and support.
  • Ability to work effectively as part of a team and interact with workers from a variety of agencies.
  • Valid drivers\xe2\x80\x99 license preferred.
Nice-to-Haves
  • Knowledge of other languages is an asset.
  • Understanding of cultural competency and anti-oppression framework;
  • Reflective of a multi-barriered group, as serviced by the program, a strong asset
