Job Summary
The Psychiatric Registered Nurse IIB (Care Facilitator) with the Mental Health and Addictions Program, Waterford Hospital, works under the direction of and reports to the Division Manager to provide leadership in the daily management of care delivery.

The Care Facilitator acts as a resource with clinical expertise for the Program and Division and promotes/coordinates an interdisciplinary team approach in the delivery of quality patient care. Responsibilities include participating in the development, implementation and evaluation of quality initiatives, leading and participating in policy development, collaborating with Educators/Division Managers/Staff to identify, coordinate, and develop staff orientation, student orientation/placements and continuing education initiatives, planning and coordinating staff replacement needs, participating in committees, providing education to patients and families and participating in ongoing education The Care Facilitator also formulates and implements treatment plans for patients with complex discharge needs in collaboration with the psychiatrist, treatment team and community stakeholders, attends rounds, takes a leadership role in nursing within the division, and develops and maintains strong links to community agencies. Other related duties may be required as needed.

Regularly moves or lifts between 5 - 10 lbs., occasionally moves or lifts up to 25 lbs. and occasionally is required to push patients in wheelchairs over 50 lbs.
Regularly walks, stands or sits to provide care or to observe patients.
Regularly uses fine finger/precision work requiring steadiness and accurate movement/control.
Visual concentration is required to observe a patient\'s behavior and body language.
Occasionally, required to drive to patients homes for visits or to accompany them to community agencies or resources.

Job Qualifications
Graduation from an approved/recognized post-secondary nursing education program with a Baccalaureate degree in Nursing or equivalent education or training is required
Practicing licensure with the College of Registered Nurses of Newfoundland and Labrador (CRNNL).
Active certification in BCLS.
Minimum of five (5) years\' experience within the last seven (7) years in adult acute care mental health is required.
Demonstrated progressive leadership experience is required.
Demonstrated strong organizational, interpersonal, team building, communication and decision making skills.
Demonstrates initiative and ability to work independently.
Demonstrated facilitation skills with staff and student clinical initiatives.
A satisfactory record of work performance and attendance is required.
