Responsibilities
Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
Monitor patients' health by taking vital signs such as blood pressure, pulse, and temperature; report any concerns promptly
Support medication administration following care plans and HIPAA privacy regulations to ensure confidentiality and safety
Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
Maintain a clean and safe environment by performing light housekeeping tasks such as cleaning, laundry, and organizing personal spaces
Observe and document patient conditions, behaviors, and progress to inform ongoing care strategies
Use specialized equipment like Hoyer lifts or resident's to assist with mobility or transfers safely
Skills
Proven experience in home & community care or assisted living environments; group home or nursing home experience highly valued
Knowledge of dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or autism spectrum disorder
Certifications such as CPR, First Aid, HIPAA compliance training preferred; willingness to obtain necessary certifications required for the role
Strong patient observation skills with the ability to monitor vital signs and recognize changes in health status
Ability to perform heavy lifting safely when assisting residents with mobility or transfers using equipment like Hoyer lifts
Excellent communication skills for working effectively with patients, families, and healthcare teams
Experience in medication administration, creating or following care plans, and providing direct support tailored to individual needs
