Join THP's Innovation & Partnership Projects team to help advance our mission of delivering a new kind of healthcare for a healthier community. Reporting to the Director of Partnership Projects and Project Directors, the Project Coordinator supports strategic initiatives across portfolios such as seniors' health, digital health, mental health, and health equity. You'll collaborate with project managers, clinical teams, and leadership to coordinate project activities, track progress, and contribute to planning and execution. Ideal candidates bring strong organizational skills, attention to detail, and a passion for person-centered care and innovation.

Innovation and quality improvement are key enablers to achieving Trillium Health Partner's (THP) mission of a new kind of healthcare for a healthier community as outlined in the organization's 2019-29 Strategic Plan. Reporting to Project Directors, the Project Coordinator will work alongside Project Managers, Directors, point of care staff and other team members to support exciting innovative initiatives, processes, and best practices that introduce novel models of care and solutions across the organization.


This includes enabling strategic and operational planning for multiple portfolios of work including but not limited to seniors' health, digital health, mental health, health equity, to name a few. Key Responsibilities:



In collaboration with Project Director(s), Project Manager(s) and other team members, support all aspects of projects across the life cycle including planning, execution, monitoring, control, and closure. Assist in the execution of project deliverables by focusing on key coordination activities. Take the lead on coordinating meetings and creating work-back plans to ensure that project-related deliverables are on time. Provide input into regular updates on project progress, milestones, and potential issues. Escalate risks as appropriate. Assist with organizing and facilitating project meetings, workshops, and briefings with stakeholders. Assist in the preparation and presentation of project materials including proposals, project plans, meeting agendas and meeting action items. Liaise with leadership and team members across various work streams to enable integrated communications and ensure successful completion of deliverables. Work directly with Project Directors, Project managers and other leaders to develop agendas and materials for meetings and ensure that key decisions are captured through minute taking and action items are followed up on. Contribute to performing environmental scans and based on information collected, making recommendations to support decision making. Support development or relevant project reporting including status reports and reports to THP staff and donors.

Qualifications & Skills:



Successful completion of a university degree in business, clinical/health informatics or health-related discipline. Experience in project management preferred. Effective communication and interpersonal relationship skills are required. Experience in health care environments such as hospitals, government or private sector is preferred. Project Management Professional (PMP) designation preferred but not required. Experience working with point of care clinicians and leadership is an asset. Excellent organizational, time management and project management skills with demonstrated ability to multi-task and prioritize tasks effectively. Exceptional attention to detail, ensuring accuracy and consistency in documentation, scheduling and project tracking. Effective analytical and problem-solving abilities to assess project data, monitor progress and support informed decision making. Ability to synthesize and present information in a concise manner that is easily understandable and engaging for the intended audience through presentation decks, pitches and other project materials.
