Motivates, directs and coaches RNs and RPNs to ensure high quality of care consistently meets defined organizational standards
Collaborate with operations leadership to ensure the implementation and sustainability of exemplary operational practices
Collaborate to address quality, safety and risk management issues, including the development of improvement plans
Collect and assess key performance indicators for the programs and initiate quality improvement/process changes as required
Lead program development and design of clinical workflows that support new models of care delivery for the target populations, in collaboration with the operations leadership team and system partners
Prepares monthly schedule for nursing field staff, understanding available resources for program/service and keeps quality in the forefront of all resource utilization decisions
Leads or assists with the integration of client satisfaction initiatives; ensure that all referred client concerns and complaints are satisfactorily addressed and documented in a timely manner
Develop educational initiatives and support the ongoing development of our nursing teams
Supportive of developing best practice Champions within the nursing team
Qualifications
Current CNO registration in good standing as a Registered Nurse (RN), degree preferred.
Minimum of 5 years of recent experience with progressive responsibilities in a supervisory role, preferably in a community setting
Experience managing in a unionized environment
Solid knowledge of the principles, practices, and methods of community nursing and service delivery to patients as well as program development, implementation and evaluation
Valid G driver's license and reliable vehicle
