Active Care Youth and Adult Services (ACYAS) is a progressive, innovative organization operating residential resources for youth, and adults in British Columbia. Position Overview
Reporting to the Director or designate, the Complex Care Manager is responsible for providing oversite, support, and supervision to service delivery and operational staff who are delivering twenty-four (24) hour services to adults who may experience one or more of the following: long-term homelessness, has been unable to maintain housing, substance use, brain injury,
mental health disorders, co-occurring disorders, previous or current justice involvement, and/or street entrenchment. The Complex Care Manager works in collaboration with the service delivery partner to create and maintain a cohesive, collaborative, and transparent working relationship. Provides crisis intervention support to staff following traumatic incidents and in addition, to clients in the absence of the clinical team. Manages and mitigates risk and works to develop a safe and healthy environment for clients, staff, and the community.
Primary responsibilities:
Oversee the day-to-day operations of the complex care program, working collaboratively and transparently with service delivery partners.
Work closely with the service delivery partners to create a co-hesive and collaborative environment for both staff and clients.
Conduct team meetings involving all team members from both service delivery providers.
Develops and manages a collaborative process for conflict resolution and decision making with the service delivery partner.
Works in partnership in with clinical team to review referrals, intakes, and overall planning.
Work collaboratively with the clinical team and staff to assess individual client risks and develop safety plans
Intervene and manage crisis, provide support, and counseling to clients in the absents of the clinical team on an emergent basis.
After hours on-call support for emergencies and critical incident debriefing.
Establish an environment that supports clients to engage in the program and to take pride and ownership in their home.
Understand and integrate risk management strategies to reduce risk and ensure the health & safety of clients, staff, and the community
Provides critical incident debriefing, immediately following an incident. Applies trauma informed care to staff's response to grief, loss, and traumatic incidents and to clients I absence of the clinical team.
Review all critical incidents for appropriate and professional documentation and forwards incidents to the appropriate individuals. Provides feedback to staff on writing clear, concise, and factual critical incidents as required.
Conduct a monthly analysis of all internal and external critical incidents and noteworthy events looking for themes and patterns and provides the supervisor with a report that includes strategies for improvement.
Provide orientation and annual training and additional professional development opportunities to ensure staff can competently complete their over job duties.
Use a trauma-informed, inclusive, equitable and diverse coaching and mentoring approach to engage, support, and supervise staff.
Hire, manage performance, and terminate staff in accordance with the Collective Agreement.
Foster staff's personal learning, growth, and career goals, which include but is not limited to completing probation and annual performance evaluations.
Create and manage the staffing schedule and ensure there is sufficient staff to provide the required staffing levels.
Conduct documentation audits and provide feedback on areas that require improvement.
Provide oversite, training, feedback, and support to all areas of medication management, ensure that all policies and procedures are followed.
Conduct monthly medication audits and analysis of medication errors looking for themes and patterns, providing written reports that include strategies for improvement.
Build a high performing team with a focus on inclusion, equity, diversity, and leadership.
Build positive relationships with community members and collaborate proactively with the neighbors to reduce and prevent conflict.
Manage conflicts with neighbors and the community and use conflict resolution strategies aligned with a restorative justice approach.
Develops and implements written program procedures and has input into program policy.
Provide operational oversite to ensure the household runs smoothly, meets health and safety expectations, and ensures maintenance upkeep.
Manage all financial aspects of the day-to-day operations including expenses, staff timesheets and expenditures, and ensures alignment with the program budget.
Provide input into the program performance measurement goals, enters the required data and monitors progress in monthly meetings with the Complex Care Leadership Team.
Other duties as required
Required Qualifications:
Social Work (Registered), Registered Psychiatric Nurse, or Human Services degree
Five (5) years of recent related experience and a minimum of (2) years of supervisory experience
Cultural knowledge of Indigenous populations and an understanding of truth and reconciliation principles.
Current First Aid
Class 5 Drivers License
Or a combination of education, training, and experience
