POSITION:

Program Manager Complex Medical Needs

WHAT YOU NEED (QUALIFICATIONS):

Maintain and utilize a current BLS Certification Provide clear criminal record and child intervention checks. Maintain active registration with the College of Licensed Practical Nurses of Alberta (CLPNA) or College and Association of Registered Nurses of Alberta (CARNA) 5 years working with individuals with complex medical needs Minimum of 3-5 years of nursing and supervisory experience Maintain a Class 5 driver's License with a minimum of 1 million dollars Liability insurance.

WHAT YOU'LL BE DOING? (JOB DUTIES):

Responsible for the recruitment, coaching, performance management and on-going supervision of Program Coordinators and other assigned positions. Provides clinical consultation services to medical professionals and family members of individuals who are experiencing concurrent complex medical health problems during transition and discharge planning. Ensure all program sites comply with health and safety standards, accreditation standards and all relevant legislation. Stays current on all relevant health concerns, coordinates and corresponds with medical professionals and other health care professionals to ensure that all health needs are addressed in a timely manner, identifying when additional support may be needed. Leads the intake process, ensuring that individuals are matched to appropriate services and that funding is in place to meet needs of each individual.
