Program Facilitator - TNO/GenTO to

Training as Recreation Therapist, Activity Aide, Program Worker, Physio Therapy Aide, Social Service Worker, PSW
or similar is required for this role, and the ideal candidate will have
2-3 years of experience working with vulnerable clients.
Required qualifications include
a current Recreation Therapy, Social Service Worker, Social Worker, Physio Therapy Aide, P.S.W. certificate, diploma, degree
or equivalent, proficiency with MS Office, and
experience in program activation, recreation, and virtual programming
. Preferred qualifications are a
Physio Therapy Aide or Activation Diploma
, and
experience with frail or at-risk seniors
, particularly those with
cognitive or mental health issues
. Essential skills include
strong interpersonal, communication, organizational, and problem-solving abilities
, with the capacity to work independently and as part of a team, manage challenging situations, and apply health and wellness concepts in therapeutic programming.
Customer service skills, an understanding of proper body mechanics
, and certifications in Food Handling, CPR, and First Aid are assets.
Experience in a Day Centre or community service setting
is also preferred.
Duties & Responsibilities include but are not limited to:
Providing safe and effective direct care and support during programs in order to meet the needs of program members as identified in the interdisciplinary care plan
Supporting the independence, dignity, social, emotional and cultural well-being of Program participants
Promoting an environment that is conducive to dementia care, mental health care and physical frailty in a community day program setting
Supporting the delivery of in-person related to health and wellness of Program participants
Planning and conducting assigned programs as well as initiating new programs and planning of special events under the guidance of Therapeutic Recreation
Initiating and maintaining therapeutic relationships with the Program participants, their family, and other care providers
Provide advocacy and act as a liaison with other service providers
Design group session programs and activities promoting health and well-being
Ensure client's physical safety and emotional well-being as well as all Social
Ensure compliance with all health and safety policies
Ensure all clients and the community feedback and concerns are addressed in a timely manner and recorded according to TNO's policies and procedures
Ensure program guidelines are followed in order to achieve successful outcomes
Serve as a liaison between program clients, participants and community networks
Work with the Seniors Program team in achieving successful outcomes
Plan, organize and facilitate group workshops and orientation sessions
Plan, prepare and evaluate appropriate material for group facilitation & workshops in collaboration with other teams and programs within the agency
Input data into appropriate database, maintains required program records, and submits required weekly and monthly reports
Coordinate activities with other TNO departments and services
Participate in networks of neighbourhood-based service providers
Actively participates as a team member and works with others
Together with other members of the team, participates in the set up, clean up and take down of the daily activities
Plans, delivers, participates and evaluates all programs and services for this role
Vaccination Requirement
- You must be fully vaccinated against COVID-19, subject to any valid medical or religious exemption from vaccination as provided by the Ontario Human Rights Code.
Screening
- As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.
Job Type: Part-time contract (14 hours/week) until March 31, 2026.
Schedule: Tuesdays and Thursdays from 9:30 AM to 5:30 PM.
