HCSS is looking for an experienced PSW to join our Day Away Program at our Meaford location as a Program Assistant. The candidate will be a Qualified PSW that is passionate about providing care to seniors and adults with disabilities. This position will be 75 hours biweekly. The Program Assistant will also work directly with clients in the Adult Day Relief Program, including assisting clients with personal care and nursing care as delegated by Registered Staff.
DUTIES AND RESPONSIBILITIES:
Responsibility for Clients:
Assist with activities of daily living including grooming, dressing, bathing, toileting assistance, mobility and transfers
To follow client service plan demonstrating professional and competent work ethic
Reporting changes in client behavior and condition
Maintaining relevant client documentation
Establishing respectful and therapeutic client/caregiver relationships
Promoting client independence and enhancing their quality of life
Maintaining professional boundaries with clients/caregivers
Responsibility for Team:
Be respectful and show a professional and cooperative spirit with all members of the multi-interdisciplinary team.
Participate in team meetings.
Communicate relevant information to the Team regarding clients, behaviours, condition and changing needs
Participate with all team members in assessing client's needs and evaluating planned interventions/programs.
Assist with orientation and training of new team members
Work collaboratively with partners in care ie. Community Case Coordinators
Responsibility for Family Support:
Communicate with families of clients as needed.
Responsibility for Administration:
Ensure daily documentation is completed.
Participate with all team members in review of the client care plan.
Immediately report any health risks or unsafe circumstances to the Team Leader
GENERAL RESPONSIBILITIES:
Proven ability to cope with and properly handle emergency situations
Seeks seminars, workshops and continuing education opportunities related to position
Ability to work independently
To seek guidance from Team Leader whenever in doubt
Good organizational and time management skills
Maintains privacy and confidentiality
Complies with the Ontario Health and Safety Act and its associated regulations
Completes mandatory annual online training courses

QUALIFICATIONS:
P.S.W. certification
Minimum two years experience working with the elderly and adults with physical disabilities.
Ability to contribute on a multi-functional team.
Current driver's license and reliable vehicle.
Ability to work independently
Knowledge and experience with computer software, cellphones and related technology
