Job Summary




Southlake Health's Cardiovascular Prevention and Rehabilitation Program delivers a structured 6-month rehabilitation experience designed to support cardiac patients in their recovery, lifestyle changes, and long-term health outcomes. We are currently seeking a knowledgeable and motivated Professional Resource to join our team in a clinical leadership role.



This position plays a vital role in overseeing program operations, guiding quality initiatives, and ensuring excellence in patient care. The Professional Resource acts as a leader, liaison, and resource for the interprofessional team, with responsibilities spanning clinical, operational, and strategic domains.

Qualifications




Education

B.Sc./B.A. Honors Degree in Exercise Science, Kinesiology, Physical Education, Physiotherapy, or related health care field required Advance skills and certification in Exercise Therapy for cardiovascular conditions preferred ACSM (American College of Sports Medicine) Exercise Specialist CSEP, CEP Courses in ECG interpretation, exercise testing, chronic/cardiac disease required Current BCLS with AED certification required


Experience

Minimum of 5 years' experience in cardiovascular rehabilitation required Extensive experience in case management/coaching for cardiovascular disease clients required


Professional Affiliations/Memberships

Membership with the Canadian Association of Cardiac Rehabilitation Membership with the Canadian Society of Exercise Physiologist - CEP or equivalent Registered with a recognized regulated healthcare college

Primary Responsibilities



Provide clinical leadership and expertise to the cardiovascular rehabilitation team Review and triage program referrals to ensure timely and appropriate patient intake Liaise with the Program Manager to coordinate daily operations and support long-term operational planning Communicate program data and volumes to CorHealth, and ensure alignment with reporting standards Participate in Ministry of Health forums and quality improvement initiatives Analyze program data in collaboration with the Manager to inform decisions, monitor performance, and improve patient outcomes Liaise and collaborate with community stakeholders and partners to enhance care coordination and program reach Coordinate scheduling for staff and physicians, ensuring adequate coverage and operational continuity Support equipment oversight, patient flow, and resource utilization Contribute to program planning, development, and growth through a forward-thinking and visionary approach Promote a culture of teamwork, innovation, and best practice in cardiovascular prevention and rehabilitation Support program operations including CPET testing, facilitation of initial patient consultations, conduct track trials, develop and implement individualized exercise prescriptions, and weekly program follow-up Collaborate with the Manager to support community-based programs and ensuring consistency in service delivery and alignment with program standards * Collaborate with the Manager and interprofessional team to organize daily huddles and team meetings and program level Clinical Support Team meetings (CST)
