This is a very exciting time to join our team. Thirteen new First Nations led Primary Health Care Centers are in the process of hiring and beginning operations across the province, with very diverse models of service delivery. In addition, the Oral Health team has recently joined the Primary Health Care program, and the Virtual Services program is at the beginning stages of implementing team based care in a virtual environment. A creative, flexible person with experience and expertise in primary care change management will find this a very exciting role.

Title:

Primary Health Care Practice Facilitator

Salary:

$86,778 - $102,091 - $117,405 per year (pay range explained below)

Contract:

Temporary - contract term until December 31, 2026

Hours:

37.5 hours a week, 8:30am - 4:30pm (core hours 9am-3pm Mon-Fri)

Location:

Metro Vancouver office - Alberni office, on the ancestral Coast Salish Territories of the Musqueam, Squamish, and Tsleil-Waututh Nations. Hybrid working in line with FNHA's flex work policy. (Onsite/Hybrid - minimum 50% in the office).

About the role



As Primary Health Care Practice Facilitator, you will play a critical role in driving quality improvement and change management within FNHA's primary health care services. By leveraging collaboration, feedback, and evidence-based practices, the Facilitator enhances the delivery of culturally safe, trauma-informed care across both virtual and in-person settings, while supporting the integration of FNHA systems with provincial initiatives. This is a term role and will work with other colleagues on a Primary Health Care Implementation Supports team to support co-creation of strategies and practical implementation plans that will lead to comprehensive, team based, culturally safe primary health care models.

About you



To be successful in this role you will have:

Education



Baccalaureate degree or associate degree in business, healthcare management or another related field. Change management certification required.

Experience:



A minimum of 5 years of experience with change management and program implementation Experience in primary health care is required. Experience in change management with Primary Care teams is required. Experience with integration of provincial EMRs (preferably MOIS) into existing workflows would be an asset. Experience working with BC First Nations communities and/or organizations in an administrative or management capacity is an asset.



Knowledge:



Proven skill in facilitating quality improvement. Demonstrated skills in initiating and managing change when rolling out new programs and changes to teams. Ability to be flexible, adaptable and thrive in a dynamic, evolving environment. Advanced understanding and interpretation of data reports and the ability to translate data into valuable practice level QI projects. Strong oral, presentation, and written communication skills

The Pay Range is the minimum and maximum annual salary based on full time equivalent hours.

Incumbents are typically hired, transferred or promoted between the minimum and midpoint of the range based on their knowledge, skills, abilities and experience in relation to the role requirements.

The top 10% of the pay range is for the incumbents who are industry experts in the job with the combination of exceptional experience and competencies needed to perform all duties and responsibilities at a superior capability level. Internal employee compensation will be determined as per the FNHA Employee Total Compensation Policy.
