POSITION: Primary Care Transformation Lead
UNIT: Elgin Ontario Health Team (Elgin OHT)
STATUS: Full Time
SALARY: $50.61 - $59.21
The Elgin Ontario Health Team (Elgin OHT) is made up of organizations, clinicians, and community members working together to improve the health and wellbeing of the people of Elgin. Together, we are reimagining how to provide care in a more integrated, coordinated way that better serves and supports our community. We're looking for a systems-thinker who has a passion for and understanding of primary care, population health, health equity, and quality improvement, with the ability to motivate others and work directly with clinicians and community members to impact change.
Through collaborative leadership, project management, and interprofessional engagement, the Primary Care Transformation Lead will support and guide the advancement of integrated, team-based primary care within Elgin, in partnership with the Elgin Primary Care Network (PCN). They will support the implementation of key OHT integrated care priorities, with a focus on primary care. They will be a key member of the OHT Staff Team, and work in close partnership with OHT Clinical Lead(s) and Elgin PCN leadership.
WHAT YOU WILL BE DOING:

  • Collaborating with the OHT Clinical Lead(s) and Elgin PCN leadership to align planning, engagement, and implementation of OHT/PCN activities
  • Lead and support primary care transformation initiatives that align with Elgin OHT priorities
  • Provide strategic and operational support to the Elgin PCN in alignment with the province's Primary Care Action Plan and the goals and objectives of the Elgin PCN
  • Facilitate strong engagement with the local primary care community and collaborative partners
  • Apply project management, change management, and quality improvement methodologies
  • Work with regional supports and/or provide practice facilitation to help the primary care community adopt quality improvement approaches and digital tools
  • Use data and evidence to inform planning, monitor progress, and evaluate outcomes
  • Coordinate communication, primary care engagement efforts, and progress reporting
  • Aligned with the OHT's role in population health management and integrated care, assist the OHT Staff Team and partners in leading and delivering on other projects, initiatives, and deliverables as required (e.g., through integrated care strategizing, project management expertise, conducting environmental scans, drafting business cases and proposals, and communication updates)
WHAT YOU MUST HAVE:
  • Degree in health administration, nursing, public health, or other field with related experience and proven successes.
  • Knowledge of Ontario's health system.
  • Strong knowledge of primary care and the province's platform for primary care transformation (including PCNs)
  • Demonstrated ability to lead change
  • 5+ years of health system leadership
  • Demonstrated ability to lead and manage projects with multiple partner involvement
  • Demonstrated creativity and innovative, solutions-focused approach
  • Strong relationship-building, communication, and facilitation skills
  • Experience working with physicians and interdisciplinary teams
  • Comfort using data for planning, quality improvement, and decision-making
  • Exceptional organizational skills with the ability to develop project plans and manage projects with multiple partners and constantly adjust priorities
  • Demonstrated excellence in interpersonal, written and verbal communication skills with the ability to demonstrate tact and diplomacy with diverse groups, including community members, front-line healthcare providers, community agencies, and healthcare administrators
  • Advanced knowledge of computer applications (e.g. Microsoft Office, Zoom) and willingness to learn new applications (e.g. Smartsheet)
  • Demonstrated ability to build partnerships and foster collaborative relationships
  • Demonstrated ability to collect and analyze quantitative/qualitative data and apply insights for continuous improvement opportunities
  • Demonstrated effective presentation skills including the ability to present and discuss complex information in a way that establishes rapport, persuades others and gains understanding, including in a virtual environment
  • Commitment to equity and advancing the Quintuple Aim
  • Ability to regularly attend work in accordance with the Attendance Support Program
  • Acceptable performance record that is aligned with STEGH values and Code of Conduct
WHAT WE PREFER YOU HAVE:
  • Resident of Elgin (St. Thomas or Elgin County) and/or strong knowledge of the Elgin community
  • Experience with practice facilitation and quality improvement in primary care.
  • Interest in or knowledge of digital health enablers (e.g. EMRs, virtual care, patient portals)
  • Project or change management certification (e.g. PMP)
  • Quality Improvement training and/or exposure (i.e. Lean Six Sigma)
HOURS AND WORKING CONDITIONS:
  • Hybrid role with regular in-person engagement across Elgin
  • Flexible scheduling; must have access to personal transportation
