LOCATION




Located approximately 100 kilometres east of Toronto, NHH is a vibrant community hospital delivering a broad range of acute, post-acute, outpatient and diagnostic services. Directly serving a growing population of more than 60,000 residents spread across a mix of urban and rural settings, NHH directly employs approximately 750 people and relies on the additional support provided by a highly engaged and supportive network of physicians, midwives, and volunteers.




ROLE OVERVIEW




The

Primary Care Project Coordinator

will play a key role in advancing primary care integration and coordination across the OHT-N. Reporting to the OHT-N Executive Lead, the Coordinator will support collaborative planning, project implementation, and data-informed decision-making related to primary care priorities and initiatives.


This role is ideal for someone who thrives in a fast-paced, evolving environment and is passionate about supporting system transformation through strong relationships, thoughtful coordination, and a deep commitment to equity and quality care. The Primary Care Project Coordinator position is a Monday - Friday, hybrid position. It is work-from-home with regular on-site meetings within Northumberland County, and travel to recruitment events/conferences.




KEY RESPONSIBILITIES



Project Coordination & Implementation




Support the development, execution, and monitoring of OHT-N's primary care workplan and initiatives. Facilitate timelines, deliverables, and reporting requirements. Track progress against goals and support continuous quality improvement.

Interest Holder Engagement




Serve as a liaison between primary care providers and OHT-N partners, ensuring strong communication and collaboration. Organize and support working group meetings, including agenda preparation, minute-taking, and follow-ups. Build and maintain strong relationships with local primary care teams, community providers, and health system partners.

Data & Evaluation Support




Support the collection, analysis, and visualization of data to inform planning and decision-making. Assist in developing performance indicators and evaluating project outcomes.

Communications & Knowledge Sharing




Develop clear and engaging communication materials for internal and external interest holders (e.g., newsletters, updates, briefing notes). Maintain shared resources, tools, and documentation related to primary care initiatives.

System Integration & Innovation




Assist with the coordination of new models of care and partnerships to support access, attachment, and equity in primary care. Support OHT-N participation in provincial primary care initiatives and learning collaboratives.





QUALIFICATIONS/REQUIREMENTS



Post-secondary degree or diploma in health administration, public health, social sciences, or a related field. 3+ years of experience in a project coordination or system planning role, ideally in healthcare or the community sector. Strong knowledge of Ontario's health care system and primary care context (e.g., family health teams, CHCs, solo practices, etc.). Exceptional organizational, interpersonal, and communication skills. Demonstrated ability to engage diverse stakeholders and build trust. Experience with project management tools, data analysis, and virtual collaboration platforms (e.g., MS Teams, Excel, SharePoint, SurveyMonkey). Ability to work independently and collaboratively in a hybrid environment. Passion for health system transformation, equity, and person-centred care.





ASSETS




Experience working within or alongside primary care teams. Familiarity with Ontario Health Team structures and goals. Understanding of health equity principles and community engagement best practices. Knowledge of Northumberland County's local health and social service landscape.

EMPLOYMENT REQUIREMENTS




Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.


Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
