Delivering health promotion, disease prevention, and chronic disease management programs in the primary care office setting, as a member of the primary care interprofessional team. The Primary Care RN will work daily with staff and physicians to assist patients with their health. The RN will report to the Director of Programs & Quality Initiatives.
Duties Include:
Well Baby/Well Child Visits
Immunization Program
Chronic Disease Injections
Acute Care/ER avoidance
Hypertension treatment
Qualifications:
Registered Nurse in good standing with the College of Nurses of Ontario
Impeccable assessment skills of all ages but especially newborn to 5 year old
Knowledge of normal development and behavior in the newborn to 5 year old
Gain knowledge of resources to refer patients to and follow up with plans
Have or be willing to gain knowledge in breast feeding support
Have or willing to gain knowledge in the administering of immunizations and educating the patient
Broad knowledge of chronic diseases (cancer, diabetes, bowel disease) in order to help educate patients and administer their medications
Knowledge and skill to perform paps and STI testing
Basic nursing skills (BP monitoring, removal of sutures/staples, wound management, ear syringing)
