The Opportunity
We are looking for Prenatal Outreach Support Team Nurse to join our Health Team at CUPS for a 12 - 18 month term maternity cover role.
As a CUPS Prenatal Outreach Support Team (POST) Nurse, you will be responsible for providing trauma-informed nursing care to clients who are potentially experiencing the adverse effects of poverty, trauma and chronic illness. Within this role specifically, you will focus on supporting clients seeking pre- or immediate post-natal support, both within the CUPS clinic setting and in the community where our clients live. In your role, you will work to the full scope of your practice and assess, plan, implement, evaluate and document all nursing care of patients in accordance with policies and professional nursing standards.
You will play a primary role in supporting an integrated, holistic and client-centered approach to patient care, health promotion and maintenance. The role will support the creation of a safe and trauma-informed health environment for clients and their families. You will provide quality patient care while promoting the shared mission, vision and values of CUPS.
LOCATION: Onsite, CUPS - 1001 10 Ave SW, Calgary
REPORTS TO: Team Lead, Clinical Services
CONTRACT TYPE: Temporary, 12 - 18 months maternity cover
SCHEDULE: Full-time, Mon to Fri, 40 hours per week
STARTING SALARY: $83,000.00 - $87,382.00
Key Responsibilities:
Work collaboratively with roles in partner organizations that, alongside CUPS, comprise the POST team. This includes Kindred Case Managers and a Calgary Police Service (CPS) Officer.
Participate in triaging incoming referrals, including setting priorities and facilitating referrals to appropriate team members, providing insight into client health care needs and fit for program.
Plan and deliver direct, low barrier health care services to clients who are marginalized and who may have complex needs and may not otherwise seek pre- or post-natal support, in both clinic-based and outreach capacities.
Champion women and gender diverse clients' health needs and family planning through advocating for appropriate client health care and social supports and support clients in the navigation of the health care system.
Help ensure continuity of care across the continuum by liaising with interdisciplinary team members, coordinating and implementing clinical interventions, evaluating outcomes based on client's goals for care, coordinating ongoing care and supporting transitions.
Provide health teaching including preventative health concepts concerning sexually transmitted and blood borne infection (STBBIs), harm reduction, nutrition, personal hygiene, pre-natal care and well-baby care.
Conduct regular prenatal check-ups and physical and mental health assessments, monitoring the health status of both mother and fetus throughout the pregnancy, and identifying and addressing potential pregnancy complications promptly.
Administer necessary medications and prenatal vitamins; perform required tests including ECG's, phlebotomy and point of care tests.
Assist in the development and implementation of individualized birth plans, prepare mothers for labor and delivery and provide guidance and support with postpartum care and breastfeeding.
Develop and maintain strong external collaborative connections with partnering agencies including but not limited to hospitals, CPS, Children & Family Services, and shelter partners.
Attend and conduct client home visits.
Transport clients to appointments when necessary to support health needs and client goals.
Employment Requirements:
Completion of an accredited nursing education program.
Active registration and practice permit with the College of Registered Nurses of Alberta (CRNA) or College of Licensed Practical Nurses of Alberta (CLPNA).
3-5 years' experience working in an acute care environment; experience working with prenatal/postpartum clients would be an asset.
Current basic First Aid & CPR certification.
Well-developed clinical and physical assessment skills including experience with mental health assessments.

Intermittent physical activity including walking, standing, sitting, lifting and supporting patients.

Use of personal vehicle to visit clients in the community and occasionally transport clients to appointments where required.
