Our diverse range of services include workplace training and career services, community volunteerism, IT training, Life Coaching, Speech courses, supported recreation, and residential services. Our clients include people from all walks of life and experience, looking to move ahead in employment, personal goals, or who just want to make connections and volunteer in their community. Our programs and services are open to people 16 years of age and older living with a disability, mental health needs, FASD, chronic illness, brain injury, or sensory impairment. We offer a unique range of options for individuals who may require support or transitional services.
Several of our community programs and services benefit from additional funding generated by the two Bottle Depot centers Cosmos operates in Red Deer and our recently opened Blackfalds location. All our depots support countless bottle drives for community charities and service organizations. We also have several drop box locations around the city for our customers' convenience.
Job description:
Assists individuals with identifying recreational, leisure, volunteer and educational activities.
Facilitate outings.
Determine an individual's desires and abilities.
Consider peer compatibility, age appropriateness, safety concerns, individual, group contract hours and monthly expense allotment.
Developing and distributing individual calendars.
Encourage participation in activities by acting as a role model.
Assist and facilitate in the preparation and provision of equipment and supplies necessary for activities, i.e.: tickets, sports equipment, craft supplies, baking goods, etc.
Assist and facilitate in the booking of facilities and transportation requirements, i.e.: gym, curling rink, bus, etc.
Participates alongside the individuals being supported. Taking part in activities such as swimming, bowling, and outdoor activities inclement weather.
Develops and maintains volunteer and educational sites. Administer assessments.
Develop placements as required.
Complete the required documentation, i.e.: placement job analysis, placement procedure checklist, task analysis.
Provide on-site tours to determine compatibility.
Ensure individuals, guardians, residential support workers are aware of placement expectations regarding dress codes, hygiene, work habits, skills, hours of volunteering, etc.
Provide transportation training to and from placements.
Provide on-site training and establish natural support for fading.
Provide guidance to enhance social independence. Accessing opportunities to further challenge individual's capacities.
Provide support and guidance in accessing the community, i.e.: restaurants, recreational facilities, public buildings, etc.
To provide holistic support, in a person-centered manner, that includes supporting each person's right to explore and be educated about sexuality, and to subsequently enjoy a lifestyle unique to their values.
Monitors individual hours and expenditures Documentation and submission of individual hours and expenses individual hours and expenses.

Prepares and delivers individual reports and completes required documentation. Write daily log-notes based on goals, activities, and participation.
Prepare and distribute quarterlies based on individual goals upon request.
Develop and present service plans.
Assist with the development of behavior management strategies, reports and programs.
Establish and maintain individual files.
Complete paperwork for individual referrals and facilitate process

Attend team, agency and committee meetings as schedule permits
Promotes individual empowerment and independence by offering choices, teaching self-advocacy, and problem-solving skills. Update and maintain knowledge regarding community resources and activities.
Establish and maintain effective communication with clients, co-workers, guardians, Community Service Coordinators, residential agencies, volunteer placements, and all stakeholders.
Provide emotional support to individuals to assist them in expressing their feelings, desires, and frustrations.
Always maintain high ethical and professional standards and practices.
Be an active team member and remain open to suggestions. Empower team members through positive feedback and utilizing strengths and expertise. Participate in the facilitation of change, transition, and conflict resolution with individuals and teams.
Perform other duties assigned related to the daily operation of Cosmos Community Support Services Ltd. This may include, but not be limited to, working in other departments including the depots to assist in the delivery of quality service.
Education: Secondary School (Required)
Experience: Support Care -2 years (Preferred)
Hours of work: 7:00am to 3:00pm
