Post Secondary Transition Facilitator (bilingual)

Job Description

Inclusion NB (formally the New Brunswick Association for Community Living) is seeking a dynamic, energetic and enthusiastic individual committed to the vision, values and philosophy of inclusion. The facilitator will assist students and young adults with an intellectual or developmental disability to make successful transition to NBCC/CCNB and other post-secondary with a focus on employment following graduation.

Responsibilities:



Assist with the decision-making process of students and young adults interested in attending community college through the Special Admissions process; Assist the students in identifying supports needed for post-secondary; Advise on the preparation of Individual Learning Plans (ILP's) and modifications for Special Admissions students; Assist in the preparation of application(s) for funding; Ensuring proper supports are in place; Monitoring of supports and progress of students; Assist in setting up work placements and provide information on job development strategies; Provide on site consultation through classroom observation and feedback to instructors; Provide formal training and workshops to NBCC/CCNB staff personnel; Meet with school districts and school personnel to promote post-secondary opportunities; Deliver presentations to families in regions throughout New Brunswick regarding Special Admissions and consultation support services.

Qualifications:



Bilingual: Written and Spoken French and English mandatory

University degree in Social Sciences (or related area) or Community College (Social Sciences) Diploma with a minimum of 2 years of experience, or an equivalent combination of education and/or minimum of 6 years of work experience on behalf/or for the disability sector. Excellent time management skills and ability to multi-task and prioritize work; Strong organizational and interpersonal skills; Strong written and verbal communication skills; Results oriented; Demonstrated leadership ability; The ability to travel and provide own transportation; Have the ability to travel around the province and be away overnight. Lived experience or professional development courses and/or training in career transition services is considered an asset.
