Multimorbidity, or the presence of two or more non-communicable chronic diseases in an individual, may be one of the greatest challenges facing health care. The prevalence of multimorbidity is high, with 30% of Canadians 40 years of age or older having multimorbidity and the burden of multimorbidity increases significantly with age and social vulnerability. Multimorbidity is too often viewed through a biomedical lens; when in truth, many chronic conditions such as hypertension, type 2 diabetes and obesity have strong social determinants. Personalized approaches that consider the complex interaction between clinical and situational factors in multimorbidity hold considerable promise in addressing the challenge of multimorbidity. Shared decision-making (SDM) is a method through which patients and clinicians can work together to develop personalized solutions in the face of medical complexity. While SDM is strongly recommended for patients with medical complexity due to multimorbidity, it is under-utilized and sub-optimally supported, particularly in racialized patients.
The Canadian Collaboration for Complex Care (C4) will develop personalized health solutions by focusing on the promotion of situational literacy. We define situational literacy as an enhanced understanding of the interaction between the person, the complexities of their lived experience and the complexities of their clinical conditions. We propose that this understanding is fundamentally important to developing effective personalized care plans for patients with multimorbidity. Our overarching goal is to demonstrate a transformative and transportable approach to designing tools and environments that promote personalized decision-making.
