Position Summary
The Physiotherapist (PT) is an integral member of the Physiotherapy service, Allied Health, clinical programs. Through the provision of patient and non-patient care activities, the PT enhances the quality of life of patients by planning and implementing individually designed programs of physical treatment to maintain, improve, or restore physical functioning and cardio-respiratory health, alleviate pain and prevent physical dysfunction in patients.
UHN's Transitional Care Program (TCP) includes UHN's Buttonwood Reactivation Care Centre (RCC), Hillcrest Reactivation Centre & Queens Estate Transitional Care units. The RCC aims to improve restoration to help patients maintain social, cognitive and physical functioning by leveraging patient-specific, goal-based service planning. Patients serviced by the TCP do not require acute care but need to engage in activities to improve their function and independence, provided less active medical management and more therapy and activity.
