About Us
The Lower Columbia Community Health Centre Network Society is a not-for-profit organization committed to providing holistic, accessible healthcare to all - including the vulnerable populations we serve. Located amidst the stunning natural beauty of the Kootenay region, our organization owns and operates a new community health centre in Trail, BC. We are looking to add to our multidisciplinary team that delivers comprehensive care addressing the physical, mental, and social determinants of health.
Your Role
Providing community-based physiotherapy care to patients of the Lower Columbia CHC. You will be part of an interprofessional team including Nursing, Occupational Therapists, Dietitians, Social Work, Counsellors, Respiratory Therapists, NPs, Pharmacist, and FPs providing team-based care to a diverse patient population.
Key Responsibilities
Assessment and Diagnosis:
Evaluate patients' physical limitations and movement patterns to identify the underlying causes of their problems.
Treatment Planning:
Develop individualized treatment plans that address the specific needs of each patient, incorporating exercises, manual therapy, and other interventions.
Treatment Delivery:
Provide physiotherapy treatments
Patient Education:
Educate patients about their condition, treatment options, and how to manage their symptoms and prevent future problems.
Progress Monitoring:
Regularly assess and document patient progress, making adjustments to the treatment plan as needed.
Collaboration:
Work collaboratively with other healthcare professionals, such as doctors, nurses, and occupational therapists, to ensure comprehensive patient care.
Health Promotion:
Promote healthy lifestyles and encourage physical activity to prevent injuries and maintain overall well-being. Group classes for common issues.
Documentation:
Maintain accurate and detailed records of patient assessments, treatments, and progress.
Referrals/Interprofessional Consultations:
Review cases and utilize skills of team members to optimize patient care.
Continuing Education:
Stay up-to-date with the latest advancements in physiotherapy through continuing education and professional development.
Qualifications and Skills
Current registration with the College of Health and Care Professions of British Columbia (CHCPBC), formerly College of Physical Therapists of British Columbia (CPTBC).
Class 5 Driver's License
