This role will be part of Specialized Orthopedic Developmental Rehab team, as a temporary full time contract of 6 months. Key Responsibilities:

  • Partner with clients and families to formulate and implement individualized client-centered goals with focused, targeted and measurable outcomes.
  • Provide client care in collaboration with the client/family to plan, implement and evaluate care, documenting the habilitative/rehabilitative process according to hospital policies.
  • Interface and problem solve with members of the interprofessional care team and community providers as appropriate to ensure an integrated approach to care.
  • Incorporate evaluative activities into professional practice to ensure the delivery of efficient and effective health care.
  • Utilize research to establish and support evidence-informed practice.
  • Utilize the teaching/learning process to provide education on various conditions, both formal and informal, to clients/families, colleagues, students, and other professionals within the organization and the community.
  • Provide direct service, consultation, mediation and education to clients, families, and community providers with focus on maximizing the client\xe2\x80\x99s physical functioning.
  • Promote and support the physical, psychosocial and spiritual well being of patients and families.
  • Practices according to organizational policies and standards for practice as set out by the College of Physiotherapists of Ontario.
  • Participates in quality improvement initiatives, supports the maintenance of a safe and healthy work environment and advances a culture of client/patient safety through work and daily practices.
  • Other duties as assigned
Qualifications:
  • Masters degree in Physiotherapy (or Bachelor degree in Physiotherapy plus other Masters)
  • Current registration with the College of Physiotherapists of Ontario
  • Membership in the Canadian Physiotherapy Association and/or the Ontario Physiotherapy Association is an asset.
  • Knowledge of and skills in physical therapy assessment and treatment of children and youth with a variety of conditions
  • Knowledge of and experience with physical therapy assessment techniques, therapeutic skills, modalities and their applications.
  • Experience with current outcome measures (i.e. Canadian Occupational Performance Measure, Goal Attainment Scaling, and WeeFIM)
  • Demonstrated ability to manage fluctuating clinical responsibilities effectively and efficiently
  • Demonstrated ability to communicate effectively with clients, families and members of the interprofessional care team
  • Proven ability to independently and collaboratively as a member of an interprofessional team is essential
  • Demonstrated commitment to professional development
  • Status appointment in the Department of Physical Therapy at University of Toronto is an asset
