Pulmonary rehabilitation (PR) is a comprehensive intervention for patients with chronic respiratory disease who are symptomatic and have difficulty with activities of daily living. Our Pulmonary Rehabilitation program includes patient assessment, exercise training, education and psychosocial support. PR is recommended for patients with chronic obstructive pulmonary disease (COPD) and is beneficial for individuals with other chronic lung diseases, notably inter\xc2\xadstitial lung disease (ILD), lung cancer, pulmonary arterial hyper\xc2\xadtension and those who are pre- or post-lung transplantation.

Position Summary:

  • Works in partnership with the RT coordinator to provide comprehensive Pulmonary Rehabilitation services across Trillium Health Partners
  • Shares in program planning, continuous quality improvement and evaluation
  • Provides physiotherapy assessment, goal oriented treatment interventions, client/family education and support to address medical and emotional needs
  • Performs as a coach/mentor, consultant and uses sound critical thinking skills to achieve client outcomes and facilitate team functions
Qualifications:
  • Baccalaureate degree required
  • Current certification of competence from the College of Physiotherapists of Ontario is required
  • Current BCLS required
  • Minimum of 2 years acute care medical/surgical experience required
  • Current certification as a Certified Respiratory Educator (CRE) an asset
  • ADP authorization preferred
  • Experience with chronic disease management and prevention
  • Experience with adult education principles and teaching in groups
  • Demonstrated ability to provide patient/family centered care
  • Strong interpersonal, organizational, prioritization and problem-solving skills
  • Demonstrated ability to work independently and as a member of an interdisciplinary team
  • Evidence of ongoing, current professional development
  • Computer literate: EPIC, Word, Excel and Outlook
