Mount Sinai Hospital is looking for a Complex Transitions PT Practice Resource, who will be a key member of the Rehabilitation and Complex Transitions teams as part of the Health Disciplines portfolio, and a primary clinician on the interprofessional Emergency Department (ED) team. This role will help lead and support clinical practices and initiatives that align alternative level of care (ALC) strategies and promote senior friendly care. The Complex Transitions PT Practice Resource will report the Senior Manager of Rehabilitation and Clinical Nutrition, and be a key member of the Rehabilitation Department, comprised of clinicians from Physiotherapy, Occupational Therapy and Speech-Language Pathology professions. Each rehabilitation clinician supports a primary patient care program across Mount Sinai Hospital, support one another to address priorities, and are key members of the interprofessional teams. The Rehabilitation Department offers their clinicians with mentorship, professional development opportunities and resources to support clinical best practices, quality initiative and professional growth.
The Complex Transitions PT's primary patient care area is the Emergency Department (ED) at Mount Sinai Hospital and may provide clinical services across all MSH patient care programs, if required. The ED is a fast paced clinical environment aimed at providing patients with timely assessment and appropriate care. The Complex Transitions PT will assess and treat patients to address physical, functional and mobility concerns in the ED environment, and work with the interprofessional team, patients/families and community providers to support transition plans that support the patient's functional needs and recovery.
The Complex Transitions PT will work with the Complex Transitions team, including the Senior Manager of Complex Transitions, Sinai Health's ALC and Complex Transitions leadership and the Complex Transitions Occupational Therapists to lead and guide practices to support effective and timely patient transitions.
