Mount Sinai Hospital is looking for a Complex Transitions PT Practice Resource, who will be a key member of the Rehabilitation and Complex Transitions teams as part of the Health Disciplines portfolio, and a primary clinician on the interprofessional Emergency Department (ED) team. This role will help lead and support clinical practices and initiatives that align alternative level of care (ALC) strategies and promote senior friendly care. The Complex Transitions PT Practice Resource will report the Senior Manager of Rehabilitation and Clinical Nutrition, and be a key member of the Rehabilitation Department, comprised of clinicians from Physiotherapy, Occupational Therapy and Speech-Language Pathology professions. Each rehabilitation clinician supports a primary patient care program across Mount Sinai Hospital, support one another to address priorities, and are key members of the interprofessional teams. The Rehabilitation Department offers their clinicians with mentorship, professional development opportunities and resources to support clinical best practices, quality initiative and professional growth.


The Complex Transitions PT's primary patient care area is the Emergency Department (ED) at Mount Sinai Hospital and may provide clinical services across all MSH patient care programs, if required. The ED is a fast paced clinical environment aimed at providing patients with timely assessment and appropriate care. The Complex Transitions PT will assess and treat patients to address physical, functional and mobility concerns in the ED environment, and work with the interprofessional team, patients/families and community providers to support transition plans that support the patient's functional needs and recovery.


The Complex Transitions PT will work with the Complex Transitions team, including the Senior Manager of Complex Transitions, Sinai Health's ALC and Complex Transitions leadership and the Complex Transitions Occupational Therapists to lead and guide practices to support effective and timely patient transitions.

In this role you will:

Possess the requisite knowledge, skills, judgment and attitude to deliver competent and evidence-based Physiotherapy care Complete assessment, planning and therapeutic intervention for an assigned patient caseload according to physiotherapy scope of practice Document in electronic health record as per Hospital and professional college standards Supervise Physiotherapy Assistants as per College Standards Demonstrate accountability and responsibility for your decisions and actions and practices within your own level of competence Participate in interprofessional rounds and discussions Continuously gather and analyze clinical and other relevant data to evaluate the effectiveness of the plan of care and adjusts the plan as needed Lead and support complex transitions/ALC strategies and represent the Health Disciplines portfolio perspective at various internal and external committees Participate in quality initiatives and/or research to promote innovation and best practice Work collaboratively with patients, families, caregivers and the interdisciplinary healthcare team Provide health education using effective teaching strategies that takes into account the patient/family's learning needs, readiness, and motivation to learn Contribute to discharge and transition planning and other planning activities to facilitate care continuity and coordination; advocate for care needs to be met in most appropriate setting Utilize critical inquiry, critical thinking and research to develop evidence-informed practice and inform decision-making Develop expertise in your area of practice through self-reflection, self-learning, specialty certification and implementation of best practice guidelines. Be committed to patient safety and quality responsibilities in accordance with hospital vision, policies and procedures Contribute to achieving Sinai Health's strategic and operational priorities through a commitment to continuous quality improvement, patient/staff safety, professional development and continuing education Contribute to Sinai's work by participating actively in hospital committees and other groups to represent the PT perspective Assist with other department/unit responsibilities or duties, as required Mentor colleagues and new hires as appropriate Participate in teaching of students from affiliated institutions. Job Requirements:

Mandatory


Currently registered and in good standing with the College of Physiotherapists of Ontario Rostered for Oxygen Titration with comfort and recent use of these skills




Preferred


3 - 5 years of recent clinical experience in an acute care environment working with adults and older adults Clinical experience providing physiotherapy service in emergency care Experience using an electronic health record Experience participating in quality initiatives and/or clinical research

Excellent health and clinical assessment skills Patient handling and functional assessment skills inclusive of objective assessments Strong disposition planning skills and knowledge of community supports Demonstrated ability in providing quality patient-focused care and addressing the fundamental care needs of patients and their families Proven ability to make sound professional judgement and practice decisions and demonstrate effective time-management and organizational skills in a fast-paced work environment Demonstrated leadership skills Excellent problem solving and critical thinking skills Excellent oral, written and verbal communication and interpersonal skills Proficiency in English Demonstrated ability to work effectively and collaboratively with others including patients and their families Demonstrated satisfactory job performance and attendance history Computer proficiency and/or Basic to Intermediate Microsoft Office skills (e.g. Word, Outlook)
