Your Opportunity:The Physiotherapist I - Physical Therapist Interventionist / Short Term Care Manager, on the Adult Team in Home Care, works as a member of an inter-professional health care team. Integrated Home Care uses a strength-based Client Centered Case Management practice framework. Using a person-centered approach, the Physical Therapist will work within full scope of practice to develop care plan(s) and provide direct care for client(s)/family(s) by identifying their needs, focusing on strengths, and setting achievable personal health and self-care goals. In collaboration with the client(s)/family(s) and interdisciplinary team you will ensure care and services are provided and modified on an ongoing basis, based on client\'s response to interventions, incorporating stewardship into service provision. The Physical Therapist fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals and other programs/organizations involved in the provision of services.Description:As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention (i.e. prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management, referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. The PT collaborates with patient(s) and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students. You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

Required Qualifications:Bachelor\'s degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta. Additional Required Qualifications:A strong clinical background in Physical Therapy. A valid Driver\'s License and a vehicle in good repair. Ability to lift 30 lbs. You will be required to operate a laptop computer and mobile phone, and for manual handling (lifting, lowering, carrying, pushing and pulling) as a regular part of the job. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation within the last 12 months. Preferred Qualifications:One year of recent experience in Community Health Setting is preferred. Previous community related experience is an asset. Excellent interpersonal and communication skills. Ability to monitor financial accountabilities. Proven assessment, critical thinking and decision making skills.
