Your Opportunity:
Reporting to the Area and Care Manager on the Team - Integrated Home Care, the Physiotherapist Interventionist Case Manager/Short Term works as a member of an inter-professional health care team, within the full scope of their professional practice. The Physiotherapist (PT) Interventionist Case Manager is responsible for assessing, planning, organizing, reviewing and evaluating the care needs of Acute and Rehabilitation clients and their caregivers/families requiring community-based services. Fosters and promotes continuity of care and co-operative partnerships by liaising with primary care physicians, residential care facilities, acute care hospitals, and other programs/organizations involved in the provision of services. Acts as an advocate for the system and the client recognizes safeguards and advances the rights and choices of the client and serves as an active liaison between the client and others internal and external to the organization.
Description:
As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention (i.e. prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management, referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care. The PT collaborates with patient(s) and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students. You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

Required Qualifications:

Bachelor's degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta.

Additional Required Qualifications:

Completion of a University Master's degree program in Physiotherapy. Active registration in CPTA. Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) from Heart and Stroke Foundation, current within the last 12 months, required. Handling, moving and lifting heavy or awkward equipment and/or individuals are required. Physically able to perform all assigned tasks. Excellent interpersonal, communication, organizational & time management skills - able to organize many details in a time efficient manner. Ability to monitor financial accountabilities, proven assessment skills. Proven critical thinking & decision-making skills. Ability to work effectively as part of an inter-professional health care team, other staff & programs/organizations, & with clients and their caregivers/families. Ability to perform all duties in accordance with related legislation.

Preferred Qualifications:

Ability to work effectively as part of an inter-professional health care team, other staff & programs/organizations, & with clients and their caregivers/families. Current AADL Authorizer a plus. Recent work experience in a community setting 2 years of Home Care experience preferred.
