Position Summary




The Physiotherapist is responsible for the caseload of patients on the Couchiching Health at Home caseload. The allied health professional working in this role will work within the community to support our home community program. This is an exciting opportunity for regulated professionals to participate in the vision of Ontario Health Team within OSMH.

POSITION SUMMARY:



The Physiotherapist is responsible for the management of community-based patients. The physiotherapist will provide assessment, care planning and intervention for patients on the CHaH program. They (with the patient and family) set goals of care appropriate for the 16 week CHaH program. The physiotherapist, as a member of the CHaH interdisciplinary team will liaison with other providers to ensure that patients reach their potential while on the program.





This role would encompass the following:

Visits patients in their own home as assigned Follows care plans and ensures the provision of patient-focused care Delegating tasks and supervision to and Occupational Therapist and Physiotherapist Assistance (OTA/PTA) Provides cognitive, social, emotional, and spiritual support Any additional tasks as identified on patient care plan A valid driver's license and reliable vehicle are required.



PRIMARY RESPONSIBILITIES:



Familiarizing themselves with the theory and application of all currently applicable procedures and modalities of treatment that they will be involved with and be able to set appropriate goals of treatment and develop and implement appropriate treatment programs. Assessing the patient noting previous history and recording all relevant details. Reassessing and modifying treatment as indicated , upon receipt of a referral containing diagnosis and a physician's signature Ensuring patient safety while delivering treatment Providing quality patient care and participating in quality assurance as indicated. Charting/documenting all patient information in the appropriate manner and at indicated frequencies. Attending clinics, rounds, and meetings concerning patient care as indicated. Upgrading their professional knowledge and skills by attending in-services, courses, and seminars as indicated.



Performs other duties as may be assigned by the Program Manager of Homecare Modernization

Qualifications




Education:

Master's degree in Physical Therapy with evidence of ongoing professional development required. Current registration with the Ontario College of Physiotherapy required. Eligible for membership with the Canadian Physiotherapy Association required. Assistive Devices Program (ADP) Authorizer in Ontario (MOHLTC) required Bone Fit(TM) Trained: Osteoporosis Canada required GLA:D Canada - Certified Instructor preferred




Experience:

Experience working with patients of various ages with neurological & orthopedic diagnosis in the acute & rehabilitation phases of assessment & treatment required. Minimum five years community-based experience required. Experience working with patients in their own homes required. Experience working with geriatric patients and those with chronic conditions required.

Demonstrated ability to work harmoniously in a demanding environment demonstrating excellent communication and interpersonal skills, an excellent attendance records.
