Home and Community Care Support Services Champlain is one of 14 Home and Community Care Support Services organizations in Ontario. We are dedicated to ensuring our more than 64,000 patients annually receive the highest-quality health care and services at home. Our patient-care coordination teams help develop care plans for people of most ages, focused on maintaining each patient\'s independence and dignity. We also provide long-term care home placement services, and facilitate our patients\' access to the community-based services they need.

Position Summary:

The Physiotherapist plans, organizes and directs diverse programs of treatment, counseling, screening and education for HCCSS Champlain Patients in a variety of settings (in homes and retirement homes)

Primary Responsibilities:

  • Provides physiotherapy services to HCCSS Champlain patients through initial assessment, identification of problems, establishment of goals and treatment plans, implementation of physio treatment, follow-up and management of patient\'s condition.
  • Establishes priorities and treatment goals with respect to identified problems and develops methods of achieving these goals.
  • Ensures that clinical records are maintained in accordance with HCCSS Champlain Policies and Procedures and the College of Physiotherapists of Ontario.
  • Schedules patient visits according to Physiotherapy priorities.
  • Functions as a member of a multi-disciplinary team and maintains liaison with other health care professionals.
  • Acts as an education and resource person for other health care professionals.
  • Participates in committees as approved.
  • Partakes in professional development activities and maintains knowledge and ability in the realm of physiotherapy.
  • Maintains supplies and recommends equipment for department use within budget limitations.
  • Interprets the role and objectives of the HCCSS Champlain Physiotherapy Program in the community.
Education and Experience:
  • Degree or diploma in Physiotherapy and current certificate of registration with the College of Physiotherapists of Ontario
  • Related professional experience which would normally be attained over a period of two years in a Physiotherapy role
  • Related experience in a similar community-based setting is an asset
  • Current CPR-C certificate
Knowledge:
  • Knowledge of health care system and the role of the HCCSS Champlain within the system
  • Knowledge of HCCSS Champlain operations, quality, risk, and performance management principles
  • Knowledge of relevant health care legislation
  • Knowledge of HCCSS Champlain policies, procedures, strategic direction and goals
  • Understanding of HCCSS Champlain partners and contracted service providers in the community
Skills and Abilities:
  • Effective planning and organizing skills to manage multiple patients, provide information reports, and take corrective action as required
  • Ability to accurately complete required documentation, reports, forms, and recommendations
  • Working knowledge of computer software (email, internet) and Microsoft Office applications (Word, Excel, Power Point, etc.)
  • Strong interpersonal skills to work with diverse patient groups with varying levels of comprehension and language capability
  • Strong ability to manage difficult conversations/situations in a healthy and effective manner
  • Demonstrated ability to work effectively in both a team environment and independently
  • Effective communication skills to establish and maintain a wide range of contacts with professionals and organizations within the community
  • Demonstrated strong customer service skills
  • Effective listening and strong facilitation skills are necessary to work with patients and their families/caregivers/SDMs in order to ensure best possible service is provided to the patient
  • Ability to deliver information effectively in a variety of settings including one-on-one, team meetings, and to assist in delivering presentations/information regarding the HCCSS Champlain Physiotherapy program
  • A valid driver\'s license and access to a vehicle is required
  • Vulnerable Sector Check completed within the last six months

Pay Rate: Wages are in accordance with the collective agreement between Home and Community Care Support Services Champlain and OPSEU and starting salaries range from $40.409/hr. - $43.169/hr.
Shift: Occasional shifts: Monday to Friday, 8:30am to 4:30pm
Location: Can be one of 850 O\'Brien Road, Renfrew OR 1100 Pembroke Street East, Pembroke
