We are currently looking to hire a:

PRIMARY HEALTH CARE COMMUNITY FAMILY PRACTICE PHYSICIAN



1 - Part-Time, Permanent (17.5 hours per week) With Benefits

Reporting To:

Interprofessional Primary Care Manager

Purpose:



The Physician will provide comprehensive primary health care for individuals and families living in the Scarborough community, with a strong emphasis on delivering inclusive and equitable services for marginalized, vulnerable, and medically complex clients. The Physician will collaborate as part of an interdisciplinary primary care team that includes other primary care physicians, nurses, nurse practitioners, social workers, chiropodists, physiotherapists, dietitians, and various allied health providers to ensure holistic, patient-centred care for all community members.

Key areas of responsibility:



Provide full-scope, patient- and family-centered care, with focus on clients with multiple chronic conditions, mental health, addictions, and social vulnerabilities.

Lead and support complex care management, including ongoing case planning for high-intensity clients.

Offer clinical leadership through consultation, mentorship, and development of medical directives and care pathways.

Advocate for clients facing barriers such as poverty, housing insecurity, and access to medications.

Collaborate with the interprofessional team and external partners to ensure coordinated, holistic care.

Participate in health promotion, screening, and outreach initiatives tailored to underserved populations.

Practice using trauma-informed, harm-reduction, and culturally safe approaches.
Supervise and mentor medical and NP learners, contributing to teaching and professional development.

Engage in quality improvement, research, and evaluation to strengthen client outcomes and reduce inequities.

Provide on-call consultation to Nurse Practitioners and participate in extended hours/after-hours coverage.

Scope and Accountability:



The Physician will practice within their full scope in alignment with the standards of the College of Physicians and Surgeons of Ontario (CPSO) and the Canadian College of Family Physicians (CCFP). The Physician will work in close collaboration with the interprofessional primary care team, ensuring safe, evidence-based, and patient-centered care for diverse and medically complex populations.

Educational and/or Professional Qualifications:



Graduate medical degree from a recognized university

Current registration and good standing with the College of Physicians and Surgeons of Ontario (CPSO)

Membership in the Canadian Medical Protective Association (CMPA)

CCFP Certification (or equivalent demonstrated clinical experience in comprehensive family medicine)

Experience or strong interest in team-based, interprofessional care models
Commitment to ongoing professional development and maintenance of CME requirements
Experience working with marginalized, at-risk, or medically complex populations is an asset

Level of Experience:



3-5 years medical practice in an inter-professional /CHC/FHT model of care is considered an asset.

Skills and Attributes:



Strong communication skills (verbal, written, listening)

Comfortable using EMR systems (PS Suit)

Works effectively in a team environment

Culturally competent and responsive to community diversity

Aligns with agency mission, vision, and values

Strong clinical reasoning and care planning skills

Willingness to support professional development and precept learners

Ability to build and maintain professional partnerships

Ontario driver's license and vehicle an asset

Fluency in additional languages relevant to the community an asset

Significant Working Conditions:



Flexibility in hours of work to provide extended hours of service including evening and weekend

Provide on call support for Nurse Practitioners.
