women\xe2\x80\x99s and infants\xe2\x80\x99 care.

The St. Joseph\xe2\x80\x99s Healthcare Hamilton Post-Acute team provides a vital service for patients who have completed the acute phase of their care at the hospital. Patients on the ALC units are waiting to be moved either to a Long-Term Care or Transitional Bed Facility. Patients on Complex Care fall into one of the following streams: Palliative Care, Slow Rehab, Chronically Vented patients, and Bariatrics. Clinical services provided on the Complex Care and ALC units are within the scope of the Department of Family Medicine at SJHH.

The Department of Family Medicine and the Post-Acute team is looking for a highly skilled and motivated physician to join the growing team on Complex Care and ALC.

The Practice:

  • Team of 3-4 physicians, each with a rota of their own patients to follow in a continuity model
  • 62 ALC and 46 CC beds
  • Full complement of health care team providers - including OT, PT, SW, Recreational
Therapy, Behavioural Supports and more
  • Provide weekday clinical support for patient care
  • Flexible schedule for on-site rounding and supporting patient care
  • Provide telephone support for nursing questions
  • Participate in on-call rota to support after-hour calls, weekend support
  • Competitive physician stipend to augment FFS billings
Qualifications:
  • Valid College of Physicians and Surgeons of Ontario (CPSO) license required
  • College of Family Physicians (CCFP) of Canada certificate required
  • Experience in Geriatrics required
  • Experience in palliative care or hospitalist work an asset
  • Ability to work collaboratively as part of an interdisciplinary team
  • Interest in teaching an asset
  • Eligibility for appointment to the Professional Staff of the Hospital
