Case Manager
to join our growing team.
In this role, you'll work closely with patients living with chronic and complex conditions to ensure timely access to their prescribed therapies. You'll support enrollment, manage reimbursement submissions, and collaborate with their healthcare providers -- all with the overall goal of improving each patient's medication access journey.
What You'll Do
Coordinate patient enrollment into PSPs, ensuring all necessary documentation is complete Assist patients with medication access
Act as a central resource to care providers and patients
Maintain accurate, up-to-date records and case notes using tools such as Kroll
Report Adverse Events (AEs) and Product Quality Complaints (PQCs)
Support treatment adherence and case persistence to improve long-term outcomes
Participate in ongoing quality improvement, training, and professional development activities
What You Bring
2+ years of experience in case management or PSP coordination (pharmacy or healthcare setting preferred)
Strong understanding of the Canadian reimbursement landscape, including public/private payer systems and Special/Prior Authorization processes
Familiarity with Health Canada's Pharmacovigilance and privacy requirements
Proven ability to collaborate within interdisciplinary teams and engage with patients in a compassionate, professional manner
Excellent organizational, critical thinking, and communication skills
Comfortable with technology, including Microsoft Office and pharmacy software platforms (e.g., Kroll)
Self-directed, with excellent time management and multitasking abilities
Clinical knowledge across chronic and specialty conditions is an asset
