We are seeking a Staff Pharmacist who is passionate about improving health outcomes, particularly for patients managing chronic kidney disease (CKD), dialysis, and post-transplant treatment. This role blends core dispensing responsibilities with advanced clinical care, education, and collaborative care planning in line with best-practice guidelines.
As part of our growing team, you will contribute to medication safety, evidence-based therapy optimization, and interdisciplinary collaboration, all while promoting compassionate care in a out-patient pharmacy setting.
Key Responsibilities:
Clinical Services
Provide medication therapy management, focusing on nephrotoxic risks, renal dosing, drug interactions, and adherence issues.
Counsel patients on complex renal regimens.
Serve as a clinical liaison with nephrologists, dialysis clinics, and transplant teams to coordinate continuity of care.
Educate patients and caregivers on medication use, potential side effects, OTC safety, and supplement risks in renal disease.
Stay current on renal pharmacotherapy guidelines and integrate into daily practice.
General Pharmacy Operations
Accurately review, verify, and dispense prescription and OTC medications in compliance with regulatory standards.
Create and maintain computerized patient medication profiles using systems like PropelRX.
Perform medication reconciliation, adverse drug reaction reporting, and problem resolution.
Process insurance claims and manage third-party billing, prior authorizations, and patient follow-up.
Administer immunizations and support public health initiatives.
Mentor pharmacy interns, residents, and students; supervise and guide pharmacy technicians.
Monitor inventory, manage medication ordering, and ensure timely disposal of expired stock.
Qualifications:
Education: Bachelor of Science in Pharmacy required. Pharm D an asset.
Licensure: Active registration with the Ontario College of Pharmacists (Part A) in good standing.
Experience: Community or clinical pharmacy setting. Prior experience with renal patients is a strong asset.
