The SFNPLC provides primary health care services to the people of Smiths Falls, and surrounding areas of Lanark, Leeds, and Grenville.
The health care we provide is delivered by nurse practitioners and supported by allied health professionals such as registered nurses, social worker, and dietitian, as well as administrative support.
The Pharmacist at the clinic works to the full scope of practice according to the Ontario College of Pharmacists.

JOB DESCRIPTION
PHARMACIST
SUMMARY
The Smiths Falls Nurse Practitioner-Led Clinic provides exemplary comprehensive primary health care services to the residents of Smiths Falls and the surrounding counties of Leeds, Grenville and Lanark.
Under indirect supervision, and working as part of the interdisciplinary team, the Pharmacist provides high quality professional services and medication management expertise to support the client's health and wellbeing and improve health outcomes consistent with the vision of the Smiths Falls NPLC. The Pharmacist works to the full scope of practice in accordance with the Ontario College of Pharmacists.
ACCOUNTABILITY
The Pharmacist collaborates with all team members and reports to the management team. The Pharmacist must comply with the standards of practice guidelines of the Ontario College of Pharmacists.
ROLES AND RESPONSIBILITIES
Medication Optimization
The Pharmacist will apply knowledge to identify, prevent, and/or resolve medication-related problems in individual clients. This may include a comprehensive assessment of a client's medication regime synthesizing information from multiple sources to identify, prevent, and/or resolve medication-related problems. Recommendations are made to the nurse practitioner or to the client when appropriate.
Documents care provided in keeping with policies and procedures of the SFNPLC Provides drug information to the team to address questions related to individual clients or generally to support best practices within the Clinic
Team and Clinic ResponsibilitiesPromotes and participates in an interdisciplinary provider model. Collaborates with both providers and clients to ensure that appropriate medication management is provided.
Participates in the development and evaluation of policies, protocols, and procedures to improve client care and/or promote co-operative and efficient staff functioning.
Contributes to the development of program plans and develops, plans, implements, coordinates, and evaluates assigned projects in consultation with other team members.
Participates in the education of students
Ensures that required statistics are recorded and participates in their analysis
Contributes where necessary to evaluation processes
Performs other duties as assigned

QUALIFICATIONS
Bachelor's degree in pharmacy
Current Part A licensure with the Ontario College of Pharmacists
Completion of an advanced degree or completion of a hospital or community pharmacy residency or equivalent clinical experience in either setting is considered an asset
Work competently in an interprofessional practice setting
Previous related client care experience, specifically in a primary care setting preferred
Ability to prioritize, manage time effectively and be flexible in a very active work environment
Ability to operate with the highest level of confidentiality and discretion
Strong interpersonal skills
Ability to work flexible hours
Ability to travel throughout Smiths Falls and the surrounding counties of Leeds, Grenville and Lanark
Demonstrated computer skills
CPIC required
Certified Diabetes Educator an asset
Minor ailments training/knowledge required
Job Types: Part-time, Permanent
Pay: Up to $57.00 per hour
Expected hours: 18.75 - 37.5 per week
